SURRY POWER STATION, UNIT NO. 1 DOCKET NO: 50-280 REPORT NO: 82-066/03L-0 EVENT DATE: 06-02-82 TITLE OF Tile EVENT: Defeat of Auto Start of Steam Driven Aux. Feed Pump

1. DESCRIPTION OF EVENT:

On June 2,1982 with Unit One at full power, a time delay relay coil (Agastat 2BAFP) failed. The time delay relay is in a reactor protection circuit which upon a station service undervoltage condition closes the outside steam generator blowdown trip valves (TV-BD-100B, D, E) and opens one of two redundant steam supply valvec (PCV-MS-102B) to the steam driven aux. feed pump. Since the relay is in a de-energize to function circuit, it performed its intended function when it failed.

4 In order to stop the unnecessary operation forthe aux, feed pump turbine, an isolation l valve (1-MS-196) upstream of the steam supply valves was closed and placed under administrative control. In an attempt to bypass the defective relay, and thus only defeat the time delay portion of the circuit, a jumper was placed across the relay contacts. This allowed the steam supply valve to be closed and the blowdown trip valves to be opened. The steam isolation valve was reopened and administrative con- .

trol was removed from the operation of the steam driven aux. feed pump.

Approximately 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> af ter the jumper was installed, it was discovered that the jumper had defeated one train of the automatic start of the steam driven aux. feed pump on a station service undervoltage condition. This is contrary to T.S. 3.b.C and is reportable per T.S.6.6.2.b.(3).

2. PROBABLE CONSEQUENCES:

l The automatic start of the steam driven aux. feed pump upon a station service under-voltage condition insures adequate heat removal from the core. The redundant train for this function and the automatic start for the 2/3 steam generator low level condition remained unaffected. Also, the motor driven aux. feed pumps were available.

Therefore, the health and safety of the public were not affected.

! 3. CAUSE:

l The time delay relay failure has been attributed to an open circuit in its coil. i i The unsuccessful attempt to only defeat the time delay portion of the circuit with the jumper has been attributed to a misinterpretation of the circuit diagram.

4. IMMEDIATE CORRECTIVE ACTION:

The operation of the blowdown trip valves and steam driven aux. feed pump was placed t under administrative control and replacement of the defective relay was initiated.  !
