DOCKET NO: 50-281 REPORT NO: 52-026/03L-0 EVENT DATE: 06-18-82 TITLE OF EVENT: Improperly Sealed Electrical Penetration

1. DESCRIPTION OF EVENT:

On June 18, 1982, with the unit at full power , a quality control inspector discovered an improperly sealed electrical penetration (fire barrier) at the pressure boundary wall between the Unit No. 2 computer room and the instrument shop. This is contrary to Technical Specification 3.21.G.2. The penetration was temporarily sealed and declared functional. This event is reportable per '

Technical Specification 6.6.2.b(3).

2. PROBABLE CONSEQUENCES and STATUS of REDUNDANT EQUIPMENT:

The functional integrity of fire barrier penetration seals in pressure boundary walls ensures that a fire cannot spread to adjacent spaces and the integrity of the control room remains intact. Since other fire protection measures were operational and the control room remained functional, the health and safety of the public were not affected.

3. CAUSE:

)

Person:lel installing a telephone cable punched a hole through an existing penetration seal. After completing the cable installation, the penetration was not resealed nor was a fire watch posted.

[ 4. IMMEDIATE CORRECTIVE ACTION:

j The immediate corrective action was to post a fire watch.

5. SUBSEQUENT CORRECTIVE ACTION:

The penetration was temporarily sealed. Subsequently the penetration was permanently sealed and a local leakage test was performed.

6. ACTION TAKEN TO PREVENT RECURRENCE:

It is suspected that the failure to seal the penetration occurred before procedures for control of fire barrier penetrations were placed in effect. -

Therefore, existing procedures should prevent recurrence.
