- , ATTACHMENT 1 SURRY POWER STATION, UNIT NO. 1 DOCKET NO: 50-280 REPORT NO: 82-062/03L-0 EVENT DATE: 05-20-82 TITLE OF THE EVENT: Heat Tracing Failure

1. DESCRIPTION OF THE EVENT:

With the unit at 100 percent power, performance test PT- 27 revealed low amp readings for circuit 7A in panels 8 and 9 (hect tracing for unit one boric acid filter discharge line). This event is contrary to Technical Specification 3.2.C.5 and is reportable as per T.S. 6.6.2.b(2) .

2. PROBABLE CONSEQUENCES AND STATUS OF REDUNDANT SYSTEMS: _

The heat tracing circuits are intended to maintain a fluid temperature above that needed to prevent recrystallization of the dissolved boric acid and thus maintain flow through the line. Boric Acid flow was verified in the affected piping and both faulty tapes were replaced within two and one half hours, thus the health and safety of the public were not af fected.

3. CAUSE:

Cuts were found in the insulation on both tapes near Flow Transmitter FT -1110.

A boric acid leak in the flange to FT-ll10 resulted in crystallized acid build up on the insulation in the area. The mechanical damage to both tapes apparently resulted while removing boric acid crystals.

4. IMMEDIATE CORRECTIVE ACTION:

Boric Acid flow to the blender was verified. PT-18.4 was performed to verify Boron Injection Tank recirculation flow. Both tapes were replaced and returned to service within two and one half hours.

5. SUBSEQUENT CORRECTIVE ACTION:

The leaking flange was repaired.

6. ACTION TAKEN TO PREVENT RECURRE'4CE:

No further action was deemed necessary.

7. GENERIC IMPLICATIONS:

None.
