, , l A Technical Specification limiting condition of operation (LCO) action was not satisfied after the Division 1 diesel generator (DG 11) was removed from standby service to perform preventive maintenance. A Techniccil Specification LCO report had been generated on the previous shift when DG 11 was made inoperable. The re31acement Shift Superintendent and Shift Supervisor were made aware of tie required actions. The missed surveillance was due to personnel error by plant licensed operators.

Inattention to detail was detennined to be the cause of this event.

Station procedure 06-0P-1000-0-0001 was amended to incorporate a surveillance requirement trigger for the diesel generators. The procedure provides a method of completing and tracking surveillances required on a daily or more frequent schedule. The procedure change should Trevent recurrence of similar delinquent diesel generator LC0 actions.

The late verification of offsite A.C. power sources did not result in a compromise to plant safety. All emergency core cooling systems were operable and available to perform the required safety functions.

Reportable Occurrence A Technical Specification limiting co.1dition of operation (LCO) action was not satisfied after the Division 1 diesel generator was removed from standby service to perfonn preventive maintenance. Technical o Specification 3.8.1.1 action b was not performed at t,1e specified frequency on January 14, 1991. This event is reported pursuant to 10CFR50.73(a)(2)(1)(3).

B. Initial Condition the plant was in Operational Condition 1 at full power with reactor water at approximately $40 degrees r and 1028 psig.

C. Descriptien of Occurrence The Division 1 standby diesel generator (DG 11) (Ells Code: EK) was removed from standby service at 0630 hours0.00729 days <br />0.175 hours <br />0.00104 weeks <br />2.39715e-4 months <br /> on January 14, 1991 for scheduled preventive maintenance. Technical Specification 3.8.1.1 required, in part, that the of fsite A.C. power sources be demonstrated operable by verifying correct breaker alignment and indicated power availability. These verifications were specified to be performed within one hour and at least once per eight hours thereafter. The initial verification was performed satisfactorily at 0637 hours0.00737 days <br />0.177 hours <br />0.00105 weeks <br />2.423785e-4 months <br />. The subsequent verification was due at 1437 hours0.0166 days <br />0.399 hours <br />0.00238 weeks <br />5.467785e-4 months <br /> o'ut not performed until 1813 hours0.021 days <br />0.504 hours <br />0.003 weeks <br />6.898465e-4 months <br />.

This did not satisfy the Technicti Specification action time limit.

D. Apparent Cause A Technical Specification LCO report had been generated on the previous shift when DG 11 was made inoperable. This LCO report was passed to the next shift as part of shift turnover activities at approximately 0700 hours0.0081 days <br />0.194 hours <br />0.00116 weeks <br />2.6635e-4 months <br />. The replacement Shift Superintendent and Shift Supervisor were made aware of the required actions. The missed surveillance was due to personnel criJr by plant licensed operators, the Shift Supervisor and Shift Superintendent Inattention to detail was determined to be the cause of this event. An incident Review Board meeting was convened on January 15 and 16, 1991 to investigate the incident. The Shift Superintendent and Shift Supervisor were counseled on working habits and methods to ensure Technical Specification compliance.

Station procedure 06-0P-1000-D-0301 was amended to incorporate a surveillance requirement trigger for the diesel generators. The procedure provides a method of completing and tracking surveillances required on a daily or more frequent schedule. The procedure change should prevent recurrence of similar delinquent diesel generator LCO actions.

A delinquent verification for DG 11 occurred on June 20, 1989 as discussed in LER 89-009-00. Inattention to detail and excessive work load detail for the Shift Superintendent contributed to that event.

Corrective actions taken for that event included instructing all Shift Superintendents to control the amount of work performed at one time to a level that they can properly supervise.

Another previous incident of a delinquent LCO action occurred on October 31, 1985 and was discussed in LER 85-041-00. The operability of the shutdown cooling system "B" loop was not demonstrated within the required time due to an inadequate operation crew turnover briefing.

Corrective action taken for that event included a change to the Shift Supervisor turnover checksheet to ensure that required LCO actions are turned over to the next shift,

f. Safety A:sessment ine late verification of offsite A.C. power sources did not result in a compromise to olant suety. The delinquent verification found the system in the correct configuration. in additfon, the Division 2 diesel generator, Division 3 diesel generator, and all offsite A.C. power sources were operable during the incident. All emergency core cooling systems were operable and available to perform the required safety functions. Preventive maintenance of DG 11 was completed and the system declared operable at 2300 hours0.0266 days <br />0.639 hours <br />0.0038 weeks <br />8.7515e-4 months <br /> on January 14, 1991. DG 11 was inoperable for 16.5 hours5.787037e-5 days <br />0.00139 hours <br />8.267196e-6 weeks <br />1.9025e-6 months <br />.
