Technical Specification Involved: 3.3.7.1 Reported Under Technical Specification: 6.9.1.12.b Event Narrative:

on August 15, 1983, the Standby Service Water System "B" loop setpoint was found above the limits of Technical Specification 3.3.7.1. The monitor is required to be operable in modes 1, 2, 3 and when the RHR heat exchangers are in operation for shutdown cooling. No Limiting Condition for Operation (LCO) was entered as Operations personnel failed to interpret the Technical Specifications correctly.

An LCO.was entered on August 17, 1983, af ter discovery of the event. The grab samples and analyses were begun immediately. Work authorizations were already issued to recalibrate the instrument. The error has been verbally discussed with the shift supervisor involved. This is a final report. This is reported pursuant to Technical Specification 6.9.1.12.b.

The grab samples and analyses required by the Limiting Condition for Operation were not started until August 17, 1983, due to personnel error. This is reported pursuant to Technical Specification 6.9.1.12.b. Attached is LER 83-108/01 T-0 with Supplementary information.
