. Attschment to AECM-83/0274 Page 2 of 3 SUPPLEMENTARY INFORMATION TO LER 83-049/03 L-0 Mississippi Power & Light Company Grand Culf Nuclear Station - Unit 1 Docket No. 50-416 Technical Specification Involved: 3.5.2 Reported Under Technical Specification: 6.9.1.13.b Event Narrative:

On April 9, 1983, a " Diesel Generator 12 Auto Start Not Available" alarm was received. An immediate investigation revealed the pressure in the C air storage tank to be 160 psig and dropping. The pressure in air storage tank D was 210 psig. Technical Specification 4.8.1.2 requires the pressure in all diesel air start receivers to be greater than or equal to 160 psig.

Therefore, Diesel Generator 12 and its associated Division 11 ECCS system were declared inoperable.

At this time the plant was operating under an LCO due to LPCI "C" being the only operable ECCS system (see LER 83-060/03 L-0). Therefore, on April 9, 1983, at 1615 hours0.0187 days <br />0.449 hours <br />0.00267 weeks <br />6.145075e-4 months <br />, action statement "b" of Technical Specification 3.5.2 was entered due to no operable ECCS systems.

Operations personnel investigating found the motor driven air compressor overheating. The compressor was secured and the breaker opened. Valve F033E (in-line drain between the air receiver and the diesel) was found not fully closed (open approximately one half a turn allowing the air tank to drop in pressure). The valve was closed and the diesel driven compressor started in an attempt to recharge the receiver., The compressor failed to transmit air to the receiving storage tank. The motor driven compressor breaker was reclosed but tripped.

At 2015 hours0.0233 days <br />0.56 hours <br />0.00333 weeks <br />7.667075e-4 months <br /> the establishment of secondary containment began. This action was discontinued when HPCS was restored to operability at 2221 hours0.0257 days <br />0.617 hours <br />0.00367 weeks <br />8.450905e-4 months <br />.

Operation under action statement "a" of T.S.3.5.2 continued until April 18 when both LPCI B and C were declared operable.

A subsequent investigation revealed the following:

o Cause of the loss of air pressure in the air receiver tank is attributed to a slightly open condensation drain valve.

o Failure of the motor driven compressor is attributed to overheating due to lack of oil and continuous operation. The heat caused a lead gasket /lockwasher to soften allowing the pressure discharge valve set screw to back out. This rendered the compressor discharge valve inoperable and prevented the compressor from discharging properly, o The diesel driven compressor failed due to a broken unloader fitting which disabled the compressor from discharging properly. It is believed this fitting may have been broken accidently by personnel previously working in the area since a portion of the fitting protrudes outside the casing.

AECM-83/0274 Page 3 'of 3 o it was noted that certain valves between the compressor and air receivers were sticking. The control air lubricator / regulator was found ',ith no oil in its reservoir. Additicaally it was discovered that no procedures existed for calibrating or performing preventive maintenance on pressure control valves.

Both compressors have been reworked and returned to service. Division II Diesel Generator was declared operable on April 13, 1983, at 1615 hours0.0187 days <br />0.449 hours <br />0.00267 weeks <br />6.145075e-4 months <br />. The i operators daily rounds were revised to include checking and adding oil to the air compressors and air lubricator / regulators. Air header drain valves (P75F033's) will be locked closed. Calibration and Preventive Maintenance procedures will be issued for pressure control valves. All corrective action is expected to be complete by~ July 31, 1983. This is submitted as a final report pursuant to T.S.6.9.1.13.b.
