Docket No. 50-416 Technical Specification' Involved: 3.7.7'

. Reported Under Technical Specification: 6.9.1.12.b Event Narrative:

-On June 15, 1983, the operations Shift Superinten' dent reviewing fire watch checksheets discovered that.certain continuous fire watches had not been performed from 1600 to 2400 hodrs.on June 14. .

Maintenance contract personnel performed maintenance in an area which work required a continuous fire watch under-Technical Specification 3.7.7. Due to the limited number of fire watch personnel available for this shift..they decided to uncover smoke detectors-in other areas and reduce-the continuous watches in effect to hourly watches thus making those persons available for the new required continuous fire watch. Due to miscommunications between maintenance personnel, the continuous fire watches were removed without notification to the. Control Room, therefore, the smoke detectors which were uncovered were'not activated and' restored to operability.- The event was discovered by the Operations Supervisor on shift when the fire watch checksheets were turned in at 2400 hours0.0278 days
0.667 hours
0.00397 weeks
9.132e-4 months
on June 14. 'The required continuous

watches were immediately reestablished. Although continuous fire watches were not conducted as. required, the hourly. fire patrols did continue in these-a'reas. .

The cause of the' event was due to personnel error. Although the Operations Department obtains and directs personnel for hourly fire watches, contract I -maintenance crews were allowed to obtain and direct continuous fire watch personnel for all contract maintenance work. /lgLfirewatchesarestill tracked by Operations.

3 l Supervisors over fire watch personnel have been reinstructed not to pull fire l

watch postings without approval of the Operations Shift Supervisor. This is submitted as a. final report puqpuant to Technical Specification 6.9.1.12.b.
