FINAL REPORT REPORTABLE DEFICIENCY 50.55(e)

ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNIT //2 I. Descri tion of Deficienc Installation Specification 13-PM-204 requires that when positioning and fitting-up a spool to a previously erected portion of a system in an established position, the estab-lished piping shall not be pulled or be permitted to deflect from position except where approved for closure. Contrary to this requirement, Lines 2-CH-424-GCBC-20" and 2-CH-142-

" HCBA-20" were pulled excessively to facilitate -the installa-tion of Pipe Support 13-CH-424-H006. The integrity of the completed welds of the piping system was indeterminate and it was impractical to perform an analysis.

II. Anal sis of Safet Im lications Bechtel cannot determine without extensive analysis whether the above condition if'ot detected could have resulted in a safety significant event; however, this condition is con-sidered reportable based on the fact that conservative judgement indicates the condition must be repaired to elimi-nate any possibility of excessive stress.

III. Corrective Action

1. Bechtel Construction is taking the necessary action to repair (remove and reinstall) the piping system to the original'esign per NCR P-.A-1814.
2. All personnel associated with the erection of pipe spools and hangers have been instructed to follow the requirements of the Installation Specification, See attached Training Session Report.
