MANAGEMENT ON SHIFT (MOS)

WEEK STARTING: 4 9 WEEKLY

SUMMARY

REPORT PAGE I OF I During this period, four individuals were on shift: P. L. Fincher, Training Superintendent from St. Lucie Plant; R. Gouldy, Senior Engineer Nuclear Licensing Department, Juno Office; B. A. Abrishami, System Performance Supervisor, Turkey Point Staff; and E. F. Hayes, Special Assistant to the Plant Manager, Turkey Point Staff.

While on shift, these individuals reported on potential safety problems, questionable work practices, operating strengths, areas for improvement and general recommendations.

Two areas of concern were identified during the week: 1) On 10/24/87 with Unit 4 in mode 5, a procedural heatup rate limit of 90'F per hour was exceeded during the collapse of the pressurizer bubble. A cooldown of the pressurizer was in progress. In an attempt to slow the cooldown rate, heaters were turned on which resulted in a heatup of 97'F in less than an hour. The Technical Specification requirement of 100oF in an hour was not violated.

Operations management review of the event revealed that procedural enhancements were needed to caution the operator on the potential for this fast heatup rate when collapsing the pressurizer bubble. Also, additional training will be performed regarding this potential sequence of events. In addition, discussions were held with personnel involved at which time they were instructed to exercise caution during heatup and cooldown operations; 2)

On 10/25/87 with Unit 4 in mode 5, the pressurizer level was being reduced in preparation for maintenance work on PORV-4-456. At approximately 0220, indications of a void in the reactor vessel upper head region were observed. Immediate action was taken and at approximately 0250 indications of a full reactor vessel upper head region were obtained.

An Event Response Team was formed expeditiously to investigate the ev'ent, address root causes and implement corrective actions, as needed. The team is currently reviewing the event but a preliminary evaluation has found that the event did not result in any major safety consequences.

During the week several operating strengths were reaffirmed; shift briefings, shift turnover, Event Response Team, off-normal procedure usage and general procedural compliance. Two areas for improvement were noted; Plant Work Order (PWO) backlog and housekeeping. Plant Management is concerned with the Plant Work Order backlogmnd several programs for reducing the backlog and tracking effectiveness are in progress. Housekeeping is being addressed plant-wide with the following actions having been taken in the last 3 weeks: 1)

All nonessential plant changes/modifications have been halted. 2) All "satellite support facilities," {!.e., trailers, sheds, tool boxes, welding machines, etc.) not required for real-time work have been removed from the protected area. 3) Installed scaffolding has been reduced to support only work-in-progress. 4) Painting and general plant cleanup has been accelerated.

In general, overall plant appearance has been markedly improved and this effort will continue,

.augmented'by routine tours by plant management.

Action has been completed or is in progress on all problems identified by the Daily Management-on-Shif t reports.

A cooldown of = ill'Fhad occurred due to turning off the heaters while fillingthe pressurizer. The corrective action of turning the heaters back on resulted in a heatup of 97'F in less than an hour. Both conditions were discovered by the Assistant Plant Supervisor-N (4-0P-041.2).

C. Questionable work practices The Unit 4 H.CO silenced/acknowledged an alarm on%he Unit 3 control board, with no verbal communication between the operators. The Unit 3 operator however was aware of the action. Other communications throughout the shift were evaluated as effective. Information was passed in a crisp, clear and concise manner. One unsafe work practice occurred when an electrician was observed standing atop a waste basket instead of using a step stool. A stool was available in the control room.

Actions taken The PSN/APSN discussed the significance of these occurrences (Section B) immediately with the personnel involved, The Operations Supv. was briefed on the pressurizer event. A memo was written requesting that consideration be given to adding a caution to the procedure and providing additional emphasis in Training.

E. Strengths

1) The conduct of the shift briefings are a positive contribution to shift continuity. They were thorough and focused on activities and expectations for the day. The P.M. briefing was used to critique the day's accomplishments and to provide appropriate feedback to shift personnel.
2) Except as noted in report item C, communications were effective and professional.
3) Control room activities were conducted in a professional, no-nonsense fashion. The control room at all times remained organized and distractions were kept to a minimum.
4) When'I&C requested to perform a pressurizer pressure control loop calibration, the RCO reviewed and walked down the maintenance procedure (MI-41-011). During this walkdown he discovered a procedural error (incorrect valve number). This demonstrates a commendable level of accountability for the activities of others which would affect the plant operations.

The event being considered was the inadvertent formation of a void in the Unit 4 reactor vessle head.

B. Immediate safety problems The immediate safety concern was addressed during the previous shift by termination of RCS depressurization and venting the reactor head through the reactor vessel gas vent system.

Questionable work practices None observed. The event response team accomplished the as outlined in Procedure 0-ADM-011. Appropriate support teams'bjectives personnel were assembled from the various departments and these resources were effectively used to gather data and analyze for root cause.

Senior management was kept informed via telephone updates.

Walked down the plant with the on-shift NRC (intake cooling water pump area, fire pumps, EDG, cranking DG and valve CV-4-2203).

Walked the Units 3 and 4 control room panels for normal indications.

Reviewed the NPO log (3-OSP-201.3) for 10-24-87. The requirements were logged properly. No remarks were indicated for some data outside expected ranges, this was discussed with NPO.

Walked NAB, RCA (personnel hatches, HHSI PP room, CCW PP rooms, EW and NS passageway), and rad waste building.

Questionable work practices 10/26/87 at a roximatel 0450 - Unit 3 A signa was received or auto ma eup to the VCT, however no acid flow indicated in the control room. Upon immediate investigations the SNPO found a piece(s) of plywood sticks in the actuator of FCV-3-113A which was preventing the valve from opening. The pieces were immediately removed and the valve functioned properly. The SNPO indicated that wood particles and sawdust were on and around the valve which he cleaned out. At this time, QSPDS indicated void in RU upper head region. Pressurizer cold calibration level increased from 86% to approximately 97% in about 10 minutes. PSN notified Operation Supervisor and Superintendent.

The Duty Call Supervisor was also called to activate an Event Response Team in accordance with O-ADM-Ol1.

Section 5.1.2.10 of Procedure 4-OP-41,8 was used to vent the reactor vessel head.

At approximately 8:00 P.M. another void was formed in Unit 4 reactor head. This void had a volume of approximately 100 ft3 Pressurizer level was raised to 40% to collapse the void.

Void started to collapse at pre-level of 25%.

The professionalism of shift personnel has improved over what I have observed in the past. They are quiet, attentive, courteous and very responsive to abnormal situations.,
