IDENTIFICATION OF OCCURRENCE: Violation of the Technical Specifications, paragraph 3.5.B.1, in that Secondary Containment was not being maintained at all times by virtue of both doors in a Reactor Building Access being open.

This event is considered to be a reportable occurrence as defined in the Technical Specifications, paragraph 6.9.2.a.6.

Routine Startup Operation The major plant parameters at the time of the occurrence were:

Power: Core 1754.1 MWt Electric 604 MWe 4

Flow: Recirc.14.9gx10 GPM Feed 6.65x10 #/hr.

Stack Gas: 9860 pci/sec.

DESCRIPTION At about 1030 on May 3,1976, it was observed that contractor OF OCCURRENCE:

personnel, while in the process of transferring equipment through the reactor building personnel airic:k, had defeated the door interlock and had both doors open. This is a vio-lation of secondary containment integrity. Plant personnel, upon recognizing this condition, directed the individuals involved to cease the transfer and immediately close the airlock doors. It is estimated that both doors were con-currently open for approximately 3 minutes.

Upon questioning the person in charge of this operation, it was detemined that plant procedures were willfully violated.

AflALYSIS OF OCCURREliCE: Secondary containment is required to minimize any ground level release of radioactive materials which might result frem an accident. The ability of secondary containment to perfom this functicn with both airlock doors open was degraded. Considering the length of time concerned the safety significance of this event is considered minimal.

CORRECTIVE .

ACTI0ft: The contractor personnel were reinstructed on the Rx Building door interlock purpose and the seriousness of this event.

Additionally, plant management directed that the responsible supervisor in charge of this job for the contractors be ter-minated due to his lack of adherence to plant procedure.
