Enclosed please find Special Report 1-SR-92-005 prepared and submitted pursuant to PVNGS Emergency Plan Implementing Procedure (EPIP-03), "Notification of Unusual Event Implementing Actions." This report discusses a condition requiring the declaration of a NOTIFICATION OF UNUSUAL EVENT due to the loss of assessment capability of all meteorological instrumentation caused by a loss of power during inclement weather.

An investigation'of this event is being performed in accordance with the Arizona Public Service Company (APS) Incident Investigation Program and is expected to be completed by September 23, 1992. A preliminary review of meteorological instrumentation data has revealed that on at least one other occasion, there was a loss of power to the meteorological instrumentation which required the declaration of a NOTIFICATION OF UNUSUAL EVENT, but no notification was made.

The investigation will include further review of recorded data and an evaluation of the requirements for declaring a NOTIFICATION OF UNUSUAL EVENT for loss of meteorological instrumentation. APS will supplement this special report within 30 days following completion of this investigation.

NPF-74 SPECIAL REPORT 1-SR-92-005 This Special Report is provided pursuant to PVNGS Emergency Plan Implementing Procedure (EPIP-03), "Notification of Unusual Event Implementing Actions," to report a condition requiring the declaration of an Unusual Event for Palo Verde Units 1, 2, and 3 at approximately 1700 MST on August 23, 1992. The Notification was terminated at approximately 1700 MST on August 23, 1992.

On August 22, 1992, at approximately 2245 MST, Palo Verde Units 1, 2, and 3 were in Mode 1 (POWER OPERATION) operating at approximately 100 percent power when the Unit 1 Control Room identified during the performance of Surveillance Test 41ST-1ZZ16, "Routine Surveillance Daily Midnight Logs," that information could not be retrieved from the Meteorological Tower. An Auxiliary Operator was dispatched to the Meteorological Tower to obtain local data and reported that power was lost to the Meteorological Tower. Loss of power was verified and Technical Specification 3.3.3.4 Action a was entered. are available for estimating potential radiation doses to the public as a result of routine or accidental release of radioactive materials to the atmosphere.

Radiation Protection was notified of the loss of power to the Meteorological Tower. In the event of an emergency, Radiation Protection will perform dose assessments using conservative "default values" when meteorological data is unavailable, in accordance with PVNGS Emergency Plan Implementing Procedure (EPIP-14), "Dose Assessment."

Control Room personnel notified the Arizona Public Service Company (APS)

Transmission Control Center of the power outage. The Meteorological Tower is powered from the APS distribution system for Buckeye, Arizona. This power source was used during the construction of Palo Verde and is separate from the offsite power sources used to operate the PVNGS units. The Transmission Control Center dispatched a division crew to restore power.

On August 23, 1992, at approximately 0557 MST, the Meteorological Tower was declared operable following power restoration and satisfactory completion of Surveillance Test 41ST-1ZZ16, and Technical Specification 3.3.3.4 Action a was exited.

On August 23, 1992, at approximately 1700 MST, during a review of the previous night's event, the Unit 1 Shift Supervisor determined that a NOTIFICATION OF UNUSUAL EVENT was required in accordance with PVNGS Emergency Plan Implementing Procedure (EPIP-02), "Emergency Classification," due to the loss of assessment capability of all meteorological instrumentation. In accordance with EPIP-03, the NRC was notified at approximately 1711 MST of a declaration and immediate termination of a Notification of Unusual Event. All appropriate state and local organizations were notified in accordance with EPIP-03 and an NRC Resident Inspector was notified.
