0 Palo Verde Nuclear Generating Station Unit 3 Notification of Unusual Event Docket No.,50-530 License No. NPF-74

, Special Report No. 3-SR-91-007 This Special Report is being provided pursuant to Emergency Plan Implementing Procedure (EPIP-03), "Notification of Unusual Event Implementing Actions" to report a condition (i.e., a fire in the protected area lasting longer than 10 minutes) requiring the declaration of an Unusual Event in Palo Verde Unit 3 at 0928 MST on November 15, 1991. The Unusual Event was terminated at 1002 MST on November 15, 1991.

On November 15, 1991, at approximately 0910 MST, Palo Verde Unit 3 was in Mode 3 (HOT STANDBY) when a mobile crane located inside the protected area came in contact with an energized 13.8 kV power line resulting in the arcing of the crane outrigger pads to ground and the burning of the asphalt area immediately in contact with the pads. At approximately 0910 MST, the site Fire Department was notified of the fire inside the protected area. At approximately 0913 MST, the Control Room Shift Supervisor was notified by an electrical foreman via telephone that the crane had come in contact with, an energized 13.8 kV overhead line, that a fire was in progress, and that the line required deenergization. 'he electrical foreman identified the line that required deenergization as NAN-S06 to NAN-S04. At approximately 0913 MST, Control Room personnel deenergized the line by securing power to the NAN-S06 bus, which also resulted in a loss of offsite power (LOP) signal to the Train B 4160 volt Class lE bus. This resulted in a Train B LOP Engineered Safety Feature Actuation System (ESFAS) actuation. The Train B Emergency Diesel Generator (EDG) started and loaded per design.

Subsequently, at approximately 0914 MST, the Control Room was notified by radio that the 13.8 kV overhead line in contact with the crane was still energized. At this time, Control Room personnel determined that the crane was actually in contact with the 13.8 kV overhead line supplied by'he NAN-S05 bus. Control Room personnel proceeded to reenergize the NAN-S06 bus prior to deenergizing the NAN-S05 bus, thereby maintaining the availability of offsite power. At approximately 0915 MST, the Fire Department response team (i.e.,

personnel, fire engine, and command vehicle) arrived, at the scene of the fire.

At approximately 0922 MST, Control Room personnel deenergized the NAN-S05 bus, following radio confirmation with the Fire;Department Incident Commander, which also resulted in a loss of offsite power (LOP) signal to the Train A 4160 volt Class lE bus per design. This resulted in a Train A LOP ESFAS actuation as designed. The Train A EDG started and loaded per design.

At approximately 0928 MST, the Shift Supervisor received information from the Fire Department Incident Commander that the fire was out and that there were no apparent injuries. The Emergency Plan Implementing Procedures require the declaration of a Notification of Unusual Event (NUE) for a fire in the protected area lasting longer than 10 minutes. At approximately 0928 MST, the Shift Supervisor declared a Notification of Unusual Event. By approximately

l NRC Document Control Desk Special Report 3-SR-91-007 Page 2 0941 MST, on November 15, 1991, the appropriate state and local agencies were notified of the condition requiring the declaration of an NUE (i.e., fire in the protected area). At approximately 0946 MST on November 15, 1991, the Nuclear Regulatory Commission (NRG) Operations Center was notified.

At approximately accordance 'y 1002 MST on November 15, 1991, with EPIP-03 the NUE was terminated in approximately 1012 MST on November 15, 1991, the appropriate state and local agencies and the NRC Operations Center were notified of 'the termination of the NUE.

Further investigation into the circumstances surrounding this event is. being

'performed in accordance with the PVNGS Incident Investigation Program. The results of the investigation will be reported pursuant to 10CFR50.73.
