PALO VERDE NUCLEAR GENERATING STATION UNIT 2 ALERT DOCKET NO. 50-529, LICENSE NO. NPF-51 SPECIAL REPORT 2-SR-93-001 This Special Report is being provided pursuant to PVNGS Emergency Plan Implementing Procedure (EPIP-04), "Alert, Site Area and General Emergency Implementing Actions" to report a condition requiring the declaration of an Alert for Palo Verde Unit 2 at approximately 0502 MST on March 14, 1993. The Alert was terminated at approximately 0115 MST on March 15, 1993.

On March 14, 1993, at approximately 0435 MST, Palo Verde Unit 2 was operating at approximately 98 percent power in Mode 1 (POWER OPERATION) when Control Room personnel noticed pressurizer pressure and level were decreasing and energized the pressurizer heaters. At approximately 0447 MST, Control Room personnel manually tripped the reactor due to a rapid decrease in pressurizer level. At approximately, 0448 MST, valid actuations of the Safety Injection Actuation System and the Containment Isolation Actuation System were received and at approximately 0458 MST, a Notification of Unusual Event was declared in accordance with Emergency Plan Implementing Procedure (EPIP-02), "Emergency Classification."

At approximately 0502 MST, it was determined that Reactor Coolant System (RCS) leakage was greater than 44 gallons per minute and the emergency classification was upgraded to an Alert in accordance with EPIP-02. Control Room personnel entered emergency procedures, as required, to diagnose and mitigate the event. At approximately 0514 MST, the appropriate state and local agencies were notified of the condition requiring the declaration of an Alert (i.e., RCS leakage greater than 44 gallons per minute). The Nuclear Regulatory Commission (NRC) Operations Center was notified at approximately 0530 MST.

At approximately 0603 MST, Control Room personnel commenced a controlled plant cooldown. At approximately 0640 MST, the Control Room Supervisor diagnosed a Steam Generator Tube Rupture in Steam Generator 2.

At approximately 0115 MST on March 15, 1993, the Alert was terminated in accordance with EPIP-04. By approximately 01 37 MST, the appropriate state local agencies and the NRC Operations Center were notified of the termination of the Alert.

Unit 2 will remain in shutdown conditions and will transition into a scheduled refueling outage. Further investigation into the circumstances surrounding this event is being performed in accordance with the APS incident Investigation Program. The results of the investigation will be reported pursuant to 10CFR50.73.
