PALO VERDE NUCLEAR GENERATING STATION UNIT 2 NOTIFICATION OF UNUSUAL EVENT DOCKET NO. 50-529, LICENSE NO. NPF-51 SPECIAL REPORT 2-SR-94-002 This Special Report is provided pursuant to PVNGS Emergency Plan Implementing Procedure (EPIP-03), "Notification of Unusual Event Implementing Actions," to report a condition requiring the declaration of a NOTIFICATION OF UNUSUAL EVENT (NUE) for Palo Verde Unit 2 at approximately 1848 MST on May 18, 1994, due to a small fire which burned in excess of ten minutes within the Protected Area. The NUE was terminated at approximately 1848 MST on May 18, .1994.

On May 18, 1994, at approximately 1840 MST, Palo Verde Unit 2 was in Mode 1 (POWER OPERATION) at approximately 86 percent power when the Control Room received notification from Security personnel that a small fire was burning within the Protected Area in the Radiologically Controlled Area (RCA) yard north of the Unit 2 Fuel Building. The Unit 2 Fire Team Advisor, a licensed reactor operator, responded to the fire. Security also notified the Fire Department.

When the Fire Team Advisor arrived at the scene, the fire had been extinguished by Security personnel with a portable fire extinguisher. A preliminary investigation revealed that a portable light had apparently fallen or blown over. The light had been placed on some plastic-covered plywood. Heat from the fallen light ignited a small amount of the plastic and plywood. The Fire Department removed the plywood and hosed down the area. Radiation Protection personnel took air samples and frisked personnel and materials as work progressed to ensure there were no radiological problems. Damage from the fire appears to be minimal.

The fire was declared out by the Fire Department, and the NUE was terminated at approximately 1848 MST on May 18, 1994. State and local agencies were notified of the declaration and termination of the NUE at approximately 1858 MST, May 18, 1994. The NRC was notified of the declaration and termination of the NUE at approximately 1927 MST on May 18, 1994.

APS has initiated an investigation under its corrective action program to determine the cause of the fire and develop appropriate corrective actions to prevent recurrence.
