I.SUMMATEOnMondaymorning,September 3,1979,atapproximately 10:45a.m.,theChicagoBridgeaIronguyedderrickbuckled,twisted,andcollapsed duringHurricane David.Thelowerportionofthederrick(belowelevation

+192feet)didnotcollapseandwasleaningataslightanglefromthe,vertical, remaining bracedtothecontainment shieldwallbyvirtueofthehorizontal strutbraces.Theupperportionofthederrickbecamedismembered andfourmajorsectionscametorestinorontheReactorAuxiliary Building.

Aportionoftheboomremainedsuspended byguywiresfromthetopofthecontainment shieldwall.Theguywiresperformed theirintendedfunctionanddidnotfail.Pertherequirements of10CFR50.55(e),

theeventisreportable becausethederrickfailurecausedsignificant damagetosafetyrelatedstructures and, weretheytohaveremaineduncorrected, couldadversely affectthesafetyofoperations oftheplant, anddoesrequireextensive evaluation andrepair.Notification oftheincidentwastelephoned intotheNRCInspection andEnforcement OfficeonSeptember 4,1979(approximately ll:45a.m.)byMr.N.Weems,FloridaPoweraLightCompany's Assistant Manager,QAConstruction.

Afternumerousonsiteinspections, engineering isintheprocessofdeveloping anddispositioning corrective actionsforthedamagedareas.Subsequent toaforementioned inspections, onsiteconfixmatory destructive andnon-destructive testingwillbeimplemented andanyadditional corrective actionwillbeissuedbyengineering ifrequired.

Aninterimreportisbeingsubmitted whileinvestigation ofthedamagedareasisstillongoingandrepairworkisproceeding.
