Report of 10CFR50.55(e) Itan on capstan Spring Tang Cracks in PSA-1 and PSA-3 Mechanical Snubbers (#103)

Dear Mr. Keppler:

On Septmber 30, 1983, the Nuclear Regulatory Cmmission (NRC) was notified of a potentially reportable deficiency concerning Pacific Scientific capstan spring tang cracks in PSA-1 and PSA-3 mechanical snubbers. Detroit Edison's Mr. W.R. Wingfield, Quality Engineer -

Construction Quality Assurance telephoned Mr. J. Konklin of NRC Region III M report this deficiency.

Pacific Scientific Cmpany recently informed Daniel International of the subject concern. Subsequent testing of PSA-1 Shock Arrestors, at Union Electric's Callaway Station by Daniel Inter-national personnel revealed a broken capstan spring tang in four (4) of seven (7) snubbers tested. Daniel International forwarded this information to Detroit 1dison.

Upon receipt of information tlat potentially defective snubbers had been shipped to the Fermi 2 Site, Field Hanger Engineering inmedi-ately began a program to cross-correlate the serial numbers with the Edison Pipe Support Mark Numbers. A ccrr.prehensive study was conducted to develop a recm mendation. Project Design has pro-posed the following to resolve the snubber problem:

Purchase new snubbers to augment those warehoused so that five (5) " seed" PSA-l's and 3's would be available for replacernents. . The renoved snubbers could then be rebuilt and used to replace the next five (5) snubbers, progress-ively. At the end of this process, the project would ._

have five (5) PSA-l's and 3's to serve as permanent plant spares.
