I.SUMMATEOnMondaymorning,September3,1979,atapproximately10:45a.m.,theChicagoBridgeaIronguyedderrickbuckled,twisted,andcollapsedduringHurricaneDavid.Thelowerportionofthederrick(belowelevation+192feet)didnotcollapseandwasleaningataslightanglefromthe,vertical,remainingbracedtothecontainmentshieldwallbyvirtueofthehorizontalstrutbraces.TheupperportionofthederrickbecamedismemberedandfourmajorsectionscametorestinorontheReactorAuxiliaryBuilding.Aportionoftheboomremainedsuspendedbyguywiresfromthetopofthecontainmentshieldwall.Theguywiresperformedtheirintendedfunctionanddidnotfail.Pertherequirementsof10CFR50.55(e),theeventisreportablebecausethederrickfailurecausedsignificantdamagetosafetyrelatedstructuresand,weretheytohaveremaineduncorrected,couldadverselyaffectthesafetyofoperationsoftheplant,anddoesrequireextensiveevaluationandrepair.NotificationoftheincidentwastelephonedintotheNRCInspectionandEnforcementOfficeonSeptember4,1979(approximatelyll:45a.m.)byMr.N.Weems,FloridaPoweraLightCompany'sAssistantManager,QAConstruction.Afternumerousonsiteinspections,engineeringisintheprocessofdevelopinganddispositioningcorrectiveactionsforthedamagedareas.Subsequenttoaforementionedinspections,onsiteconfixmatorydestructiveandnon-destructivetestingwillbeimplementedandanyadditionalcorrectiveactionwillbeissuedbyengineeringifrequired.Aninterimreportisbeingsubmittedwhileinvestigationofthedamagedareasisstillongoingandrepairworkisproceeding.II~DESCRIPTIONTheconcreteShieldBuildingandtheReactorAuxiliaryBuildingwereattherfollowingstagesofconstructionwhenthehurricanepassedthejobsite:TheconcreteShieldBuildingcylinderhadbeencompletedtoelevation+191.42feetandtheReactorAuxiliaryBuilding,beingconstructedinastaggeredmanner,hadwallsandcolumnscompletedtotheundersideoffloorslabsatelevations+43feet,+62feetand+82feet.

I.SUMMATEOnMondaymorning,September 3,1979,atapproximately 10:45a.m.,theChicagoBridgeaIronguyedderrickbuckled,twisted,andcollapsed duringHurricane David.Thelowerportionofthederrick(belowelevation

+192feet)didnotcollapseandwasleaningataslightanglefromthe,vertical, remaining bracedtothecontainment shieldwallbyvirtueofthehorizontal strutbraces.Theupperportionofthederrickbecamedismembered andfourmajorsectionscametorestinorontheReactorAuxiliary Building.

Aportionoftheboomremainedsuspended byguywiresfromthetopofthecontainment shieldwall.Theguywiresperformed theirintendedfunctionanddidnotfail.Pertherequirements of10CFR50.55(e),

theeventisreportable becausethederrickfailurecausedsignificant damagetosafetyrelatedstructures and,weretheytohaveremaineduncorrected, couldadversely affectthesafetyofoperations oftheplant,anddoesrequireextensive evaluation andrepair.Notification oftheincidentwastelephoned intotheNRCInspection andEnforcement OfficeonSeptember 4,1979(approximately ll:45a.m.)byMr.N.Weems,FloridaPoweraLightCompany's Assistant Manager,QAConstruction.

Afternumerousonsiteinspections, engineering isintheprocessofdeveloping anddispositioning corrective actionsforthedamagedareas.Subsequent toaforementioned inspections, onsiteconfixmatory destructive andnon-destructive testingwillbeimplemented andanyadditional corrective actionwillbeissuedbyengineering ifrequired.

Aninterimreportisbeingsubmitted whileinvestigation ofthedamagedareasisstillongoingandrepairworkisproceeding.

II~DESCRIPTION TheconcreteShieldBuildingandtheReactorAuxiliary Buildingwereattherfollowing stagesofconstruction whenthehurricane passedthejobsite:TheconcreteShieldBuildingcylinderhadbeencompleted toelevation

+191.42feetandtheReactorAuxiliary

Building, beingconstructed inastaggered manner,hadwallsandcolumnscompleted totheunderside offloorslabsatelevations

+43feet,+62feetand+82feet.

ComletedActionstoIdentifDamaeAstormdamageteamwasassembled todetermine anddocumenttheextentofdamageresulting fromthederrickfailure.Theteamconsisted ofaconstruction civilengineerandtwoassistant civilengineers.

Inconjunction, theArchitect/Engineer has'isited thejobsite,bothbeforeandafterderrickremovalandcleanup,toobservethedamage,andtoconfirmthetaskforcefindings.

Areascompletely unaffected bythederrickfailurehavebeenreleasedforcontinued plannedconstruction.

Investigations areongoingtodetermine ifandwheredestructive andnon-destructive testingshouldbeperformed beforereleasing areasadjacenttoareasdamaged.Thedamagetothecontainment shieldwallwaslimitedtobentandsheareddowelsprojecting fromthetopofthecylinderwallandasmallsurfacegaugeontheexteriorwall.Themappingofthisdamagedareahasnotyetbeencompleted.

Thedamage'ithin theReactorAuxiliary Buildingwasextensive andconsisted ofpenetration offloorslabs,shattered floorbeam;spalledconcrete, andbentandsheareddowelsforcolumnsandwalls.Thereisapossibility ofsomecrackingofsomefloorbeamsandgirdersandoneexteriorwallsection.Thelocationofderrickandboom'ection restingpositions, anddetailsofobserveddamagetotheReactorAuxiliary Buildingispresented ontheenclosedsketches, SK-2998-FG 75.01to75.06.Inadditiontothenoted.damageontheattachedsketches, crackswereobservedonthesurfaceofafewelevation

+43feetfloorbeams.Thesecrackswerechippedbacktodetermine depthandwereobservedtobeonlysurfaceshrinkage cracks.hTInadditionto.the.abovestructural damageintheReactorAuxiliary BuildingIII~Ithefollowing isalistingofdamagedequipment andsupports:

+62feet.Testresultswillindicateifthereinforcing whichextendsintothesurrounding soundconcretehadbeenoverstressed duringmissileimpact.Thesecondprogramrequiresnon-destructive testingofconcretemembers,1)adjacenttodamagedfloorslabareasatelevations

+43feetand+62feet,including allthebeamsatelevation

+43feetwhichwereobservedtohaveonlysurfacecracks,and2)inanexteriorwallalongthenorthsideofthebuildingwhichhadonlyfivedayconcretestrengthwhenasectionofthyderrickhitthetopofthewall.Thepulseechomethodwasselectedandwillbeperformed byMuenowandAssociates ofCharlotte, NorthCarolina.

TheattachedFigures1and2showtheconcretememberstobeinvestigated.

Inaddition, thedamagedareasonthecontainment shieldwallwillbemappedandevaluated priortoanyrepairworkbeinginitiated.

III.CORRECTIVE ACTIONSComletedCorrective ActionsFloridaPower6LightCompany's QualityControlDepartment hastheresponsibility topreparenonconformance reports(NCR's)wheredamagehasoccurredtocompleteconstruction activities (i.e.,afterconcreteplacement)or isalistofdispositioned NCR'ssummarizing categories ofItypesofdamageandcorrective actiontobetaken.equipment.
