DearMr.Grier:

Thissupplements ourlettersPLA-202datedDecember1,1977,PLA-239datedApril6,1978,andPLA-316datedFebruary2,1979andrepresents thefinalreportrelativetothesubjectdeficiency whichwasreportedundertheprovisions of10CFR50.55(e).

Thedeficiency involveddimensionally undersized weldsonsafetyrelatedpipehangersfabricated fortheSusquehanna SteamElectricStationbyITTGrinnell, Warren,Ohioundex'echtel PurchaseOrder8856-P-3.

Thenonconforming conditions represented adeviation fromhangerdetaileddesigndrawingsandrequiredanextensive evaluation ofthe"as-fabricated" undersized welds.Investigation oftheproblemrevealedthattheproblemresulted, inpart, fromthefactthatthespecification forshopfabrication ofhangers(8856-M-209) lackedspecificcriteriafortheacceptance/refection ofwelds.Thissituation wascorrected when,onOctober18,1977,Revision11ofP.O.8856-P-3wastransmitted tothesupplierandpxovidedarevisedSpecification 8856-M-.209 whichincludedappropx'iate inspection acceptance criteria.

Havingestablished thenecessary inspection criteriaforfutureproduction ofpipehangers, anextensive reinspection programforallhangersproducedbyITTGrinnellpriortoOctober18,1977wasinstituted byBechtelOCattheSSESconstruction siteonDecember1,1977andwasdevisedtopreventthe'b'sgPENNSYLVANIA POWER8LIGHTCOMPANY<90~%gN~g Mr.BoyceH.GrierApril6,1979installation ofnonconforming hangers;toverifythesupplier's compliance tothenewfabrication andinspection requirements; andtoprovideorderlycontrolsforcontinuing construction installation activities.

Visualinspections byBechtelQCconfirmed theacceptability ofallQ-listedhangersshippedbyITTGrinnellafter10/18/77.

OnDecember12,1977,theBechtelinspection effortwasdirectedtowardsallhangersatSSESwhichhadbeenfabricated priortotheOctober18,1977date.Allweldsonpipesupportsandhangercomponents wereexaminedbyBechtelinaccordance withArticle9ofASMEB&PVCodeSectionV-1971andthefollowing ANSIStandardB31.1visualacceptance criteria:

~Unacceptable indications (1)Cracks-externalsurface.(2)Undercutonsurfacewhichisgreaterthan1/32inchdeep.(3)Weldreinforcement greaterthanthatspecified inatableinPara.127.4.2oftheANSIB31.1Standard.

(4)Lackoffusiononsurface.(5)Incomplete penetration (appliesonlywheninsidesurfaceisreadilyaccessible).

~Filletweldminimumdimensions asspecified inANSIB31.1-1973.

Asnecessary, duringtheBechtelinspection effort,whichwasundertaken on12/12/77andconcluded on3/15/78,weldswhichdidnotmeetspecification requirements werereworked.

OnMarch15,1978BechtelQCconcluded theprimaryinspection functionandITT-Grinnell quality'ontrol personnel assumedtheinspection functionon-siteemploying thesame(Bechtel) visualinspection acceptance criteria.

Priortoinitiating theinspections, Grinnell's QCProgramwassubmitted toandapprovedbyBechtelEngineering.

AttheoutsetofITT-Grinnell's inspection effort,BechtelQCheldapreparatory meetingwithITT-Grinnell andestablished BechtelQCsurveillances, reviewsandwitnessandholdpoints.(BechtelQCdocuments itsactivities onFieldInspection Reportswhicharepreparedweekly.)Thereisacomputerlistingofpipehangerswhichhasbeenmodifiedtoprovidearecordofallthesafety-related pipehangersaffected.

Thelistingprovidescolumnsdesignating thehangerswhich, asdetermined bytheBechtelsurvey, havebeenjudgedacceptable orhavebeendesignated asrequiring rework.Theserecordshavebeenupdatedtoreflectwhererework'as accomplished andthestatusoftheacceptability oftheweldsaffected.

Theserecordsreflecttheinterim(12/1/77to3/15/78)Bechtelinspection andreworkeffort.

Mr.BoyceH.GrierApril6,1979RecordsofITT-Grinnell's activities aremaintained inaccordance withitsQCProgram.TherecordsofBechtelandITT-Grinnell inspections arebeingkeptasseparatedocuments.

Bothsetsofrecordscontainsimilarpertinent information.

Inconjunction withitsinspection effort,ITT-Grinnell preparedtheattachedreportoftheresults.andananalysistoverifytheadequacyoftheSusquehanna Project's hangerswhichwerefabricated byITTGrinnellinthefulfillment ofBechtelPurchaseOrderi/8856-P-3-AC Rev.12.ThisreporthasbeenreviewedandacceptedbyBechtelEngineering whohavestated:"Insummarytheconclusions ofthereportsubstantiate theadequacyoftheQ-Listedhangersinquestionbecause:(a)AllQ-Listedhangersshippedpriorto10/18/77wereinspected forundersized weldsaswellasforcompliance withtherevisedspecification requirements forvisualweldacceptance criteria.
