I.SUMMATEOnMondaymorning,September3,1979,atapproximately10:45a.m.,theChicagoBridgeaIronguyedderrickbuckled,twisted,andcollapsedduringHurricaneDavid.Thelowerportionofthederrick(belowelevation+192feet)didnotcollapseandwasleaningataslightanglefromthe,vertical,remainingbracedtothecontainmentshieldwallbyvirtueofthehorizontalstrutbraces.TheupperportionofthederrickbecamedismemberedandfourmajorsectionscametorestinorontheReactorAuxiliaryBuilding.Aportionoftheboomremainedsuspendedbyguywiresfromthetopofthecontainmentshieldwall.Theguywiresperformedtheirintendedfunctionanddidnotfail.Pertherequirementsof10CFR50.55(e),theeventisreportablebecausethederrickfailurecausedsignificantdamagetosafetyrelatedstructuresand,weretheytohaveremaineduncorrected,couldadverselyaffectthesafetyofoperationsoftheplant,anddoesrequireextensiveevaluationandrepair.NotificationoftheincidentwastelephonedintotheNRCInspectionandEnforcementOfficeonSeptember4,1979(approximatelyll:45a.m.)byMr.N.Weems,FloridaPoweraLightCompany'sAssistantManager,QAConstruction.Afternumerousonsiteinspections,engineeringisintheprocessofdevelopinganddispositioningcorrectiveactionsforthedamagedareas.Subsequenttoaforementionedinspections,onsiteconfixmatorydestructiveandnon-destructivetestingwillbeimplementedandanyadditionalcorrectiveactionwillbeissuedbyengineeringifrequired.Aninterimreportisbeingsubmittedwhileinvestigationofthedamagedareasisstillongoingandrepairworkisproceeding.II~DESCRIPTIONTheconcreteShieldBuildingandtheReactorAuxiliaryBuildingwereattherfollowingstagesofconstructionwhenthehurricanepassedthejobsite:TheconcreteShieldBuildingcylinderhadbeencompletedtoelevation+191.42feetandtheReactorAuxiliaryBuilding,beingconstructedinastaggeredmanner,hadwallsandcolumnscompletedtotheundersideoffloorslabsatelevations+43feet,+62feetand+82feet.

'II.DESCRIPTION(continued)ComletedActionstoIdentifDamaeAstormdamageteamwasassembledtodetermineanddocumenttheextentofdamageresultingfromthederrickfailure.Theteamconsistedofaconstructioncivilengineerandtwoassistantcivilengineers.Inconjunction,theArchitect/Engineerhas'isitedthejobsite,bothbeforeandafterderrickremovalandcleanup,toobservethedamage,andtoconfirmthetaskforcefindings.Areascompletelyunaffectedbythederrickfailurehavebeenreleasedforcontinuedplannedconstruction.Investigationsareongoingtodetermineifandwheredestructiveandnon-destructivetestingshouldbeperformedbeforereleasingareasadjacenttoareasdamaged.Thedamagetothecontainmentshieldwallwaslimitedtobentandsheareddowelsprojectingfromthetopofthecylinderwallandasmallsurfacegaugeontheexteriorwall.Themappingofthisdamagedareahasnotyetbeencompleted.Thedamage'ithintheReactorAuxiliaryBuildingwasextensiveandconsistedofpenetrationoffloorslabs,shatteredfloorbeam;spalledconcrete,andbentandsheareddowelsforcolumnsandwalls.Thereisapossibilityofsomecrackingofsomefloorbeamsandgirdersandoneexteriorwallsection.Thelocationofderrickandboom'ectionrestingpositions,anddetailsofobserveddamagetotheReactorAuxiliaryBuildingispresentedontheenclosedsketches,SK-2998-FG75.01to75.06.Inadditiontothenoted.damageontheattachedsketches,crackswereobservedonthesurfaceofafewelevation+43feetfloorbeams.Thesecrackswerechippedbacktodeterminedepthandwereobservedtobeonlysurfaceshrinkagecracks.hTInadditionto.the.abovestructuraldamageintheReactorAuxiliaryBuildingIII~Ithefollowingisalistingofdamagedequipmentandsupports:1-Conduitonelevation+43feet2-Cabletrayrestraintonelevations+43feetand+62'eet3-4.16KVSwitchgear2A3-1,2,3,4,5,6,7,8-Y~~='~","I4-StationServiceTransformer2A2'g'CPh,'.,h*5-480VSwitchgear6-Pressurizerheater'buses2A3and2B3T~',4'CIh,T~ITth'h'
