Pape2Mr.WilliamT.RussellJanuary27,1988Contrarytotheabove,asaresultofaninspectionconductedonSeptember8-22,1988,itwasdeterminedthattheredundant'trainthatprovidessteamgeneratorpressureindicationwouldnotbefreeofdamagebyfireintheIntermediateBuilding,elevation253'-6".Also,theredundanttrainthatprovidesprimarycoolantmakeupwouldnotbefreeofdamagefromafireintheAuxiliaryBuilding,FireareaABBM.Theseredundantsafeshutdowntrainswouldbedamagedbyfirebecausetheonehourfirebarrierprovidedforcableenclosurewasimproperlyinstalled.Theimproperlyinstalledfirebarrierwastheonehourratedfire'rapwhichprotectsconduitR975forsteamgeneratorpressureindicationandthefirewrapofconduitsL400andL398requiredfortheoperationofchargingpump1A".ReasonofViolation:Theonehourconduitfirewrapwasprocuredasaproprietarysystemtobeinstalledundervendorsupervision.TheinstallationspecificationwaswrittenbythevendorwithaGinnasitespecificaddendumaddedbyRGEE.Theintentoftheinstallationspecificationwasforallinterferencestobewrappedwithafourinchextensionortotallyencapsulated.Addendum1tothespecificationstatedthatallsupportsandinterferencesshallbeprotectedaminimumoffourinchesmeasuredlinearlyfromthestructurebeingprotected.Diagramsofacabletraywrappedinthismannerareshowninthespecification.Onevendorsupervisorusedthespecificationasintendedandthewrapinstalledunderhissupervisionwasadequate.Theothervendorsupervisordidnotunderstandthephysicalprocessesinvolvedinheattransferthroughthesupportsandinterferencesandinterpretedthespecificationtorequirethatonlythefourinchofwraparoundtheconduitwouldprovidefourlinearinchesofwrapfromtheconduit.ThesupportswrappedunderhissupervisionwereinmostcasesnotadequatetoprotecttheconduitforonehouroftheASTME119fire.Threespecificcausesforthisoccurrenceare:Lackoftrainingofvendorsupervisorinbasicphysicalprocessesinvolvedintheinstallation.2.Lackofsufficientmargininthevendorsdesigntoaccommodatediverseracewaysupportconfigurationsfoundinthefield.3.Lackofdiscussionintheinstallationspecificationastothereasonforprovidinginsulationextensionalongsupportsandinterferences.

Page3Mr.WilliamT.RussellJanuary27,2989CorrectiveStepsTakenandResultsAchieved:ThesubjectconduitsupportsarebeingrewrappedtoprovidefullonehourprotectionfortheASTME229fire.CorrectiveStepsTakentoPreventFutureOccurrences:Theinstallationspecificationhasbeenrevisedtorequiretheequivalentoftwelveinchesofinsulationalongsupportsandinterferencesinfutureinstallations.Thisprovidessufficientmargintoassuretheonehourrating.2.Analyticalmethodshavebeendevelopedtoevaluateanyconfigurationswheretwelveinchescannotbeachieved.DateWhenFullComplianceWillBeAchieved:AllrewrappingwillbecompletebyFebruary25,2989.Violation88-24-03RoutineInspectionReport50-244/88-24,AppendixA,NoticeofViolation,states.inpart:LicenseConditionC.(3)FireProtection,requiresthatthelicenseemaintainineffectallfireprotectionfeaturesdescribedintheNRC'sFireProtectionSafetyEvaluation(SE)datedFebruary24,2979.TheSEinparagraph3.2.32requiresthatquarterlyfiredrillsbeconductedforeachshiftbrigadeandinparagraph6.2itrequiresthat'hequarterlydrillsbepreplanned.Contrarytotheabove,onNovember25-27,2988,itwasdeterminedthatthelicenseedidnotperformpreplarmedfiredrillsquarterlyforallshiftbrigades.Inthefirstquarterof2988,fouroutoffiveshiftbrigadesdidnotattendapreplannedfiredrill,inthesecondquartertwooutoffiveshiftbrigadesdidnotparticipateinapreplannedfiredrill,andinthethirdquarter,threeoutofsixshiftbrigadesdidnotparticipateinapreplannedfiredrill.
