, SPECIAL REPORT l SR-os-001-00 j STEAM GENERATOR EDDY CURRENT EXAMINATION

! (6TH REFUELING) j INTRODUCTION 4

j Inservice eddy current examination of Waterford 3 Steam Generator (S/G) tubing was l l completed by Conam Nuclear, Inc., a B&W Nuclear Service Company, on March 28, 1994. l 4

This report is submitted in accordance with Technical Specification 4.4.4.5, which i requires the complete results of this inspection be submitted in a special report  !

pursuant to Technical Specification 6.9.2, within 12 months following the inspection. l The eddy current testing included 100% full length bobbin coil examinations of each  !

steam generator. Additionally, Waterford 3 performed Motorized Rotating Pancake Coil (MRPC) examinations on the hot legs of both steam generators. This examination was  ;

focused in the sludge pile region which is subject to OD initiated stress corrosion  !

cracking and intergranular attack. The number of tubes included in this augmented  !

scope totaled 6,102 tubes for S/G #1 and 6,110 tubes for S/G #2. The tubesheet MRPC l examination scanned two inches above and below the tubesheet interface. As a result  :

of the top of the tubesheet MRPC examination Waterford 3 identified No Detectable Degradation.  !

i In response to industry concerns for upper bundle hot leg sludge deposition and potential freespan axial indications, Waterford 3 performed MRPC on 303 tubes in S/G i

1. 1 and 305 tubes in S/G #2. The upper bundle MRPC examination criteria was the

longest freespan of tubing adjacent to the partial eggcrate scallop bars. ~ This

'l included MRPC scans from the eighth, ninth and tenth partial eggerates through the anti-vibration bar (Batwing BW1) to the first vertical support intersection. The MRPC examination results identified No Detectable Degradation.

S/G tube examinations were conducted in accordance with USNRC Regulatory Guide 1.83 and ASME Section XI using Conam Nuclear, Inc. procedtre 42-EC-227, "Multifrequency Eddy Current Procedure Steam Generator Tubing MIZ-18/. or MIZ-30 Digital Eddy Current System Waterford 3." As a result of eddy current te!, ting, S/G #1 had two defective tubes which were plugged in accordance with B&W Nuclear Service Company's " Field Procedure for Rolled Plugging," Drawing, 1154835. The steam generator tube plugs were manufactured from thermally treated Inconel-690. Additionally, all tube plug locations were positively identified and video taped verifying correct tube locations.

l As a result of verifying S/G #1 tube plug locations, it was noted that a Westinghouse Inconel-690 mechanical plug was leaking from the OD of the plug at hot leg location  !

Row 121 Line 45. The tube plug location Row 121 Line 45 was previously plugged with a ,

Inconel-600 Westinghouse mechanical plug during RF #2 for a 41% through wall cold leg l indication. Subsequently, Spring 1991 (Refuel 4) the Inconel-600 hot leg plugs were j replaced with Inconel-690 (3 in each S/G) due to their susceptibility to Primary i Water Stress Corrosion Cracking. Upon investigation as to the failure of the ]

Westinghouse Inconel-690 mechanical plug, it was determined that the machining of the previous Inconel-600 mechanical plug resulted in the plug tops remaining in the l tubesheet of S/G #1.

In order to install the Inconel-690 mechanical plug at location Row 121 Line 45, l Westinghouse had to push the plug remnants up into the tubesheet. During this j process of pushing the plug remnants, the tube ID was probably scored resulting in an i inadequate seal between the tube ID and plug lands. The review of Westinghouse QC l Process Data Sheets for the other S/G #1 hot leg tube plugs did not involve pushing l the plug remnants prior to accepting the Inconel-690 plugs. As for S/G #2, the plug remnants were removed prior to the installation of the Inconel-690 mechanical plugs.

Another factor which may have led to the plug failure was that tube location Row 121 Line 45 is out on the peripheral of the bundle approximately thirteen inches from the I curvature of the bowl. The confines of the tight curvature and the outer bundle tube location made it difficult to push the plug remnants into the tubesheet. Vaterford 3 contracted B&W Nuclear Technologies to machine S/G #1 hot leg tube plug location Row 121 Line 45 and install a tapered welded plug manufactured from Inconel-690. The above stated information was documented in Condition Report CR-94-308.

RESULTS OF EXAMINATION STEAM GENERATOR #1 The eddy current testing of 9,018 tubes in S/G #1 resulted in the following analysis of indications:

. 101 Tubes Less Than 20% Thru Wall

. 39 Tubes Greater Than or Equal to 20% But less Than 40% Thru Wall

• 2 Tubes Equal to 40% Thru Wall (Plugged)

• 1 Low Row Tube Obstructed Between 7H and 70 Row 2 Line 10 (Plugged)
