,-Q INTRODUCTION The preoperational examinations of the reactor vessel, reactor vessel closure head, steam generators and pressurizer for the Waterford 3 Steam Electric Station were performed after their respective hydro tests in the fabrication shops of Combustion Engineering, Inc., the manufacturer of the components, in Chattanooga, Tennessee, between August of 1975.and May of ~1976. The examinations were in accordance with Section XI'of the ASME Boiler and Pressure Vessel Code. Additional examinations in accordance with Section XI will be performed at the plant site after the system hydro.
In the perfonnance of this examination, the Combustion Engineering / Auto-mation Industries (C-E/Ati) team performed the examinations as defined in IWA-2110(a) .of Section XI; Factory Mutual Engineering served as inspectors, as defined in IWA-2120 of Section XI. By this arrangment, Factory Mutual reviewed the applicable procedures, witnessed the examinations, reviewed examiner qualifications and checked for general code compliance, as speci-fied in IWA-2120 (b) of Section XI. The Factory Mutual representative was continuously informed.of the examination schedules so that he could witness any examination or operation which would be required in the performance of his function as inspector. -

A number of reportable indications .were' found during the examinations; all, however, were within the acceptance limits. of Section XI. The Preoperational Examination Program and Report is presented in five volumes:

. Volume 3 -- Report Volume 4 -- Data i Volume 5'-- Data l This volume, Volume 4. contains copies of all examination data sheets. It also contains an explanation of the numbering system used on the examination data sheets and a guide to the interpretation of the printout from the automated examination equipment.

; o The:following pages comprise all data sheets for the shop preoperational examinations. The Calibration Data Sheets for the ultrasonic examinations 4
are filed separately in Volume 5 of the Preoperational Program and Report, and each ultrasonic data sheet in this volume carries the number of the correspondi.ng Calibration Data Sheet. For each component, all data sheets are filed by general type of examination area (shell weld, nozzle weld, support lug, etc.). Within each section the data. sheets are filed in , numerical order. For the ultrasonic sheets the. numerical ordar is based on the Calibration Data Sheet numbers, and for the surface and-visual examinations the numerical order is based on the number of the examination area in question. A table before each component's sheets gives the Cali-bration Data Sheet number (s) for each ultrasonic examination area. There ' were casss in which shell welds and nozzle welds were examined using the same calibration, and the results were recorded on the same scan data , sheet. In these cases the original data sheet is filed with the shell weld, and a copy .is put in the nozzle weld section. In those cases where report-able indications .were found, the evaluation follows immediately the data sheet which records the indication.. The following discussion is given to assist in the interpretation of the basic data output of the Nuclear Reflectoscope used for the automated examinations, which was a printout. paper tape. An example of the data _Q printout and a guide to its interpretation is given in Figure 1. The-first column of five digits and the second column of four show the two coordinates I of the positioning device used, e.g., the elevation and angular position of
;the boom when the vessel shell welds are being examined. The' third column
- of seven digits shows the echo information. The first digit is the channel
-. number; for the vessel shell welds, channel #1 is 0*, channels #2 and #3 l are 45*, and channels #4 and #5 are 60*. The'second, third and fourth digits'show information about the largest received echo. The second digit is a measure of_ echo amplitude, 1 being the smallest amplitude and 9 being the largest. The depth information is given by the third and fourth digi ts. The time of the received signal which is of interest (between the front surface reflection and the back surface reflection) is divided elec-tronically into segments called gates, and, when a printout occurs, the num-ber of the gate in which the echo appears is given by the third and fourth digits. If a second echo', smaller than the first but still recordable, . appears in the same series of pulses, its information is given in the fifth.

- O transducer. The larger was of amplitude 9 in gate 6, and the smaller was of amplitude 4 in gate 23. For any particular examination, the exact signifi-cance of each. amplitude number and the approximate depth to which each gate corresponds can be obtained from the calibration sheets for that examination.
During the automated examinations, all echoes which approached the reporting level of. 50 percent of DAC appeared on the printout as amplitude 9 indica- -
'tions. The examination scans were done at an ultrasonic gain setting of twice the reference sensitivity, and indications of a size down to 20 percent DAC were recorded on the printout. All such indications were evaluated individually by the SNT-TC-1A UT Level II operator of the Nuclear RefleSto-scope. The evaluations of the Level .II operator found the great majority to be due to a slight change in the search unit-wall water path, a change in section thickness, or multiple reflections from the cladding. Other indications of amplitude 9 were found to be less than 50 percent of DAC and, therefore, not reportable. There were also cases in which readily identi-fiable geometric reflections produced large quantities of printout, and .these are discussed below. The remainder were real indications and were evaluated fully against the evaluation criteria listed in Volume 1 of the Preoperational Examination Program and Report. The real indications are listed in Volume 3 of the Preoperational Examination Program and Report and ,
included in the accompanying data sheets. O sefore each examination or set of examinations, the TuC (Transponding uitra-sonic Calibrator) was used in conjunction with the front-panel gain controls to verify that incoming pulses of various amplitudes gave the proper print-outs, and these verifications were recorded on the examination paper tapes. In addition,-during the major examinations, the locations of the limiting coordinates were recorded on the tapes. The data printout for the nozzle-to-vessel welds contains a large amount of L print caused by the geometry of the examination area. The reason for this l print is the saddle shape of the nozzle-shell boundary. When the search l unit is inserted into the nozzle far enough to cover the outboard end of l the attachment weld at its vertical extremes, the horizontal orientation of
- the search unit during its rotation will produce echoes from the OD surface of the nozzle walls which occur at approximately the same metal path depth as would echoes from defects in the weld. Manual observation of the screen during the examination was used as a double-check to ensure that all recorded printouts were of this type. A similar situation prevailed with the nozzle extensions at the inboard end of the outlet nozzles.

The substance of the error was in reversing the data for azimuthal angle and inches from flange.
