._ _. - .. m O' TOLEDO EDISON COMPANY DAVIS-BESSE UNIT ONE NUCLEAR POWF.R STATION SUPPLEMENTAL INFORMATION FOR LER NP-33 .'7-71 DATE OF EVENT: August 19, 1977 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF CCCURRENCE: Auxiliary Feed Pu=p 1-1 steam supply isolated for maintenance Conditions Prior to Occurrence: The plant was in Mode,2, with Power (MWT) = 0 and Load 1.MWE) = 0.

Description of Occurrence: At 1530 hours0.0177 days <br />0.425 hours <br />0.00253 weeks <br />5.82165e-4 months <br /> on August 19, 1977 Auxiliary Feed Pu=p Turbine (AFPT) 1-1 trip throttle valve was closed for =aintenance personnel to test stroke " Main Steam Line to Auxiliary Feed Pu=p Turbine 1-1 Isolation Valve" MS106 per Maintenance Work Order MWO 77-1585 after repacking it. Although the repacking need not have been done at this particular ti=e, it was thought that deferring the problem until no Action Statements would be entered wculd be a peer

=aintenance and safety practice. This placed the Station in the Action State ent

V of Technical Specification 3.7.1.2.

Designation of Aoparent Cause of Occurrence: The trip throttle valve was closed to allow =aintenance to stroke MS106 wnien had just been repacked. The repacking was done with the valve backseated for which MS106 recained open and was therefere not reportable.

Analvsis of Occurrence: There was no threat to the health and safety of the public or to Station personnel. The valve being strcke tested and the trip throttle valve could have been opened if needed which would have returned AFPT 1-1 to operability. AFFT l-2 was operable to autocacically supply Auxiliary Feedwater if needed.

Corrective Action: The valve stroking was completed and MS106 and AFPT l-1 trip throttle valve were returned to normal status at 1600 hours0.0185 days <br />0.444 hours <br />0.00265 weeks <br />6.088e-4 months <br /> on August 19, 1977.

Failure Data: There have been no prior occurrences of removal of an AFPT frem service to perform a valve stroking. 1 s l Y
