| | SUBJECT: FAILURE TO OBERVE CONDITIONS OF SENIOR OPERATOR LICENSE |

| | The NRC has determined, as described in NRC Inspection Report 50-188/88-01, |
| | (documenting an inspection performed March 8-10,1988) that you failed to |
| | observe the conditions of your senior operator license (No. 2158-6) under |
| | Title 10 of the Code of Federal Regulations, Parts 55.53(e) and 55.53(f). |
| | These license conditions require that you hold an active license when |
| | manipulating or supervising the manipulation of reactor controls. These |
| | conditions were made a part of each operator and senior operator license in |
| | the revision to Part 55 noticed in the Federal Register (52FR9453) dated |
| | March 25,'1987, which became effective May 26, 1987. |
| | You are reminded that at, operator or senior operator license to manipulate the |
| | controls of a nuclear. facility imposes direct responsibilities on you separate |
| | from your obligations to the facility at which you are employed. These |
| | responsibilities include, but are not limited to, the conditions of license |
| | listed in Part 55, which is reproduced in the enclosed NUREG 1262. |
| | You should ensure that you understand these responsibilities and discharge the |
| | duties of your license with greater care in the future. A copy of this letter |
| | will be placed in your docket file. |

conditions were made a part of each operator and senior operator license in
the revision to Part 55 noticed in the Federal Register (52FR9453) dated
March 25,'1987, which became effective May 26, 1987.
You are reminded that at, operator or senior operator license to manipulate the
controls of a nuclear. facility imposes direct responsibilities on you separate
from your obligations to the facility at which you are employed. These
responsibilities include, but are not limited to, the conditions of license
listed in Part 55, which is reproduced in the enclosed NUREG 1262.
You should ensure that you understand these responsibilities and discharge the
duties of your license with greater care in the future. A copy of this letter
will be placed in your docket file.
