ATTN; Mr. LJ. Callan REG!ON IV 611 Ryan Plaza Drive, Suite 400 Arlington, TX 76011 Re: License R-88, Docket S0188 This is to report a fliilure of the intrusion alarm system for the Kansas State University TRIGA Mki! Nuclear Reactor Facility.
At 1620 hours0.0188 days <br />0.45 hours <br />0.00268 weeks <br />6.1641e-4 months <br /> on 20 April 1994 a routine monthly test of the intrusion alarm system was conducted. The previous test had been on 17 March 1994. Activation of the alarm system at the Reactor Facility failed to sound an alarm at the University Police Department. The failure was quickly diagnosed as being due to the failure of an electrical relay in the alarm unit at the Police Department. The relay was replaced, the unit was returned to service, and a functional performance test was conducted by 1700 hours0.0197 days <br />0.472 hours <br />0.00281 weeks <br />6.4685e-4 months <br />.
Pending permanent changes in test procedures, now under consideration, we will perform weekly electrical functional performance tests of the alarm system, at least !
through June,1994. Ilowever, for the weekly tests, we will not require the Police Department to dispatch ofTicers to the Reactor Facility.
