In accordance with the agreement of January 6,1982 between the State of Ohio Division of Boiler Insp(ction and Cincinnati Cas & Electric Company, the National Board Audit Team has continued its audit activities at the William H.

  • Zimmer Nuclear Power Plant.

This is Interim Report No. 3 and covers the National Board Audit Team's activities at the Zirmner Nuclear Power Plant from August 7, 1982 through '

September 30, 1982. ,

Interim Report No. 1, dated May 12, 1982, covers the National Board's activities from March 1 to April 23, 1982. Interim Report No. 2, dated July 1, 1982, cocrs the National Board's activities from May 13 through June 18, 1982.

In% rim Report No. 2 (Supplement 1), dated August 6, 1982, covers the National Board's activities from June 19 through July 30, 1982.

1.0 INTRODUCTION

1.1 Charles W. Allison , Michael F. Sullivan and Robert P. Holt were the National Board Audit Team member participants on various dates during this period (August 7 through September 30, 1982). The findings, comments and proposed resolutions are as follows.

a 2.1 At the request of Cincinnati Cas & Electric Company a. meeting _

was held at the National Board of Boiler and Pressure Vessel Inspectors' office, Columbus, Ohio, August 10, 1982 to review the final typed draf t of the National Board Audit . Team's Interim Report No. 2 (dated July 1,1982) and Supplement 1 (dated August 6, 1982). Representatives were present from CG&E, the State of Ohio, the National Board, the NRC, the Henry J. Kaiser Company, and Reedy, Herbert & Gibbons Associates. Each item in the two reports was discussed, wherein CGLE committed to write a response to the reports with their proposed resolutions for compliance with enforcement and regulatory requirements.

In addition, CG&E's response to Interim Report No. 1 (dated August 5,1982) was reviewed and discussed by the personnel present.

ATTACHMENT "A" 2.2 The National Board Audit Team has attended several meetings with Cincinnati Gas & Electric (CG&E), Henry J. Kaiser (HJK) and their respective consultants (Reedy, Herbert & Gibbons Associates; Nick Kist and Associates). The purpose of these meetings has been to establish procedures and programs acceptable to all' -" "-

pa-ties for the resolution of findings identified by the National Board Audit Team.

3.0 HENRY J. KAISER COMPANY.

3.1 Henry J. Kaiser Weld Procedure SPPM 3.1.50, Rev. O.

ASME Sec.IX 3.1.1 This procedure was originally qualified ulth impact testing.

Para. Qll(b) The procedure was used on primary containment main steam -

1971 Edition W/73 penetration to seal head welds. The materials involved in-Addenda; S&L Spec. NX these welds are SA350 Gr LF1 to SA516 Gr 70. The welds 2300; S&L Spec. H-2256, are identified on HJK Drawings PSK IMS 46, 48, 50 and 52.

Supp. 7, Para. 701.2, The welds are identified as Welds MS-M1, MS-M2, MS-M3 701.3 and MS-M4. The line numbers are identified as Lines IMSOA24, IF50B24, IMSOC24 and IMSOD24.

3.1.2 ASME Section III,1973 Edition / Summer'73 Addenda, Para.

NB 2300 requires impact testing of materials within the thickness range of these weldments and base materials.

i ASME Sec.IX 3.1.3 ASME Section IX,1971 Edition /Ninter '73 Addenda, states l Para. Qll(b)1 that a separate procedure qualification is required for l each type and grade of material when impact testing is a requirement.

3.1.5 Weld Procedure Specification SPPM 3.1.50, Rev. O, was qualified using SA106 Gr. B pipe and did not specify.

Kaiser (HJK) shall review other veld procedures of this type to assure that they comply with ASME Section III requirements and provide the National Board Audit Team with documentation of this review identifying welds in the various systems I

where improperly qualified weld procedures were used and the proposed corrective action.

3.2 Henry J. Kaiser Weld Filler Metal.

, ASME Sec. III 3.2.1 As a result of a review of both historical and current NB 2420(e) weld procedures, it was noted that electrodes (10,000 NB 4122; lbs. each) of different types, i.e., 6020, 7018, produced ASME Sec. IX by the same manufacturer were received bearing the same

No. 15220 and Murex Order 92-12978 3.2.2 The National Board is of the opinion that weld material from the same heat of steel having different physical.and chemical values leaves open the possibility of issuing the incorrect electrode type for the veldments to be made.

HJK shall review weld issue slips (KE-2 forms) to determine wherelthe weld electrodes in question were used. In addition, the National Board Audit Team shall be provided with HJK's documentation of where this veld material was used and their corrective action.

3.2.3 The National Board is of the opinion that HJK shall develop a program acceptable to the National Board Audit Team for the review of all veld filler metal and certified material test reports to assure that this condition does not exist on other HJK purchases.

If this deficiency is identified on other purchases of weld material, the National Board Audit Team shall be presented with documentation identifying the nonconformance and the corrective action taken to assure this material was not used on Code systems.

SUMMARY

Ihis is a progress report of the National Board Audit Team's activities since.

the issuance of Interim Report No. 2 (Supplement 1) dated August 6, 1982. The National Board Audit Team will continue its audit and surveillante of programs, their implementation, and the ASME construction at the Zimmer Nuclear Power Plant to assure conformance with the State of Ohio and ASME Code requirements.

In addition, the National Board Audit Team shall continue to review all pro-i posed resolutions of previously identified findings and the re-verification and confirmation programs that have been presented to the Naticual Board Audit Team, the United States'NRC and the State of Ohio to assure that proper correc-tive action is completed in accordance with the State of Ohio, the United States NRC, and ASME Code requirements.

Cincinnati Gas 6 Electric Company is advised to respond in writing to each. item contained herein within fifteen (15) working days, or no later than October 29, 1982. A copy of the CG6E response should be sent to The National Board of Boiler and Pressure Vessel Inspectors.

g 1. Foreword:

This specification is intended to govern the procedures and technical requirements for Gas Tungsten Arc and Shielded Metal Arc Welding of Carbon Steel (P1). The selection of materials, the preparation for welding and subsequent treatment of these materials during fa'brication are in accordance with the applicable provisions of the ASME Boiler and Pressure Code,Section III and IX, and ANSI B31.1.

In the event of conflict between the provisions of this specification and the applicable code and all details not specifically outlined herein, the requirements of the Code shall govern.

II. Procedure:

A. Process _

l. The welding shall be Gas Tungsten Arc and Shielded Metal Arc using Manual Equipment.

- 2. Electric current shall be direct current, Straight Polarity (Electrode Negative) for GTAW. Reverse Polarity (Electrode Positive) for SMAW.

3. The manually welded joints shall be made using a multiple pass, single arc method.

DIVISION 7 - INSTALLATION INSTRUCTIONS FOR SPECIAL PIPING SYSTEMS AND EQUIPMENT 701. PRIMARY CONTAINMENT' PENETRATION FIELD WELDS

t 701.1 All field welds to primary containment penetrations for piping are to -

be in accordance with Section III of the ASME Boiler and Pressure Vessel Code, 1971, Subsection NE, including Winter 1973 Addenda.

s 701.2 Field welds to primary containment penetrations for Class A piping shall f3 qu meet the requirements of Section III, Subsection NB for Class 1 compo-nents, including impact requirements per Paragraph NB-2300.

701.7 All field welds shall be fully radiographed except when the joint detail does not permit radiography, then ultrasonics plus liquid penetrant or ,

magnetic particle examinscion of the completed weld may be submitted as allowed by NE-5000. Examination shall be done to the requirments of-NB-5000 or NE-5000 whichever is more stringent.

701.8 All butt-welds shall be ground to the requirements of Drawing M-398, t Sheet 2, to allow for UT in-service inspection per ASME Section XI.

l 701.9 Welding of penetration head fittings for electrical penetration shall be in accordance with Subsection NE of the ASME Code,Section III.

. Welds shall meet the impact requirements of NE-2300. Low metal service s temperature shall be 40*F.

702.3 Contractor shall certify that the hangers, snubbers and piping supports for Seismic Class A and B piping have been erected in accordance with the design drawings.

702.4 Piping systems and/or piping supports which are in Contractor's design scope shall account for thermal expansion of the installed piping, move-ment of attached equipment and piping, and piping system weight includ-ing weight of piping supports. Contractor shall base his design upon Cen-eral Notes (S&L Drawing M-399) and shall follow.the general techniques for piping and support system design employed in Sargent & Lundy designed sys-tems with the exception that Contractor designed systems shall not include provision for seismic design of the piping since Contractor's design scope is limited to Class C with no B seismic and Calss D piping only.

702.5 Contractor shall support his piping from existing steel or embedments designed for that purpose. Loading on e= bed =ents shall be limited to 9 KIPS per 9 running inches. Contractor shall not cinch anchor to conctete for piping support purposes without prior approval from S&L.

702.6 Contractor shall follow criteria for supporting and routing of instru-ment piping as shown on S&L Instrument Installation Details and as speci-fled in Division 8 of this Specification.

702.7 Supplement 201.9, " Criteria for Flexibility and Support Design for Supp.7i Field Routed Piping (10-6-75)" shall be adhered to by Contractor for piping within Contractor's design scope.
