SUBJECT:

STATUS REPORT AND ACCEPTED RESOLUTION OF CROSBY SAFETY / RELIEF SOLEN 0ID VALVES On August 12, 1981, General Electric reported under 10 CFR Part'21 a concern pertaining to the subject valves. Wyle Laboratories discovered during an environmental qualification test that the valves were unable to actuate with the minimum available DC voltage during a postulated loss-of-coolant accident.

Furthermore, GE discovered in CROSBY'S Model CVG-D1, solenoid gasket material will allow steam leakage into the electrical components. This memorandum is to inform you that NRR and IE have completed their review of the proposed vendor action on the subject valves.

The NRR Environmental Qualification Branch and the IE Division of Engineering and Quality Assurance have reviewed the folicwing documents pertaining to the subject valves:

1. Crosby - test procedures -
2. Crosby - test plan for enviror. mental qualification
3. Crosby - qualification test report and test data 4 Crosby - quality assurance plan
5. Crosby - test procedure for the production assembly and testing
6. General Electric Field Disposition Instructions (copy enclosed).

The corrective action proposed by General Electric snd Crosby includes:

(1) replacing the existing solenoids with new ones that have a lower pickup voltage, CONTACT: Wolfgang Laudan, IE C 9 8302020363 821230 6 ADOCK 05000397 g jCJB3 3d

Multiple Addressees' DEC 3 0 gogg (2) replacing the BUNA N gaskets with.VITON E, and (3) using potting compound in the solenoid coil cavity and access routes to obtain a good moisture seal. This is acceptable to hRR and IE (see enclosed memo from V. Noonan to R. Baer, dated December 10,1982).

The following nuclear plants were identified by General Electric as being i affected by the solenoid and seal replacement program:

1. PURPOSE The Crcsby Sdef f Rehe! Vekelm)Y solenoid IMF-2 is to be installed on all 18 SRVs (54 total solenoids). Crosby will supply field service personnel to supervise installation - ref. MR #YC593 Rev. O.
2. REOUIRED DOCUMENTS Hone. Crosby field service will deliver the GE approved Installation Instructions,
3. All IMF-2 equipment is pre-shipped to Site. "0" rings and lubricant will be hand carried by Field Service Rep. .

Procedure is contained in 2 above. G.E. QC to observe at least on an audit basis. The removed solenoidsare to be kept by Site or returned to G.E. San Jose.

Scope j This procedure is to be followed for all field installations of Crosby IMF-2 Sole-noid Pilot Valves manufactured to QAP-3325 on the Crosby HB Dual Function Safety Ralief Valve Actuators.

2. Reference l i

2.1 Crosby Quality Assurance Plan QAP-3325.

2.2 Crosby Data Sheet DS-A-63790, Sheet 4 of 4.

2.3 Crosby Data Sheet DS-C-66181.

3. Handling and Cleaning 3.1 The pilots should remain in the protective plastic bag until ready for instal-lation. Particular care should be exercised to avoid damage to the "0"-Ring scaling surface ("0"-Ring No's 10 and 23 on DS-A-63790).

3.2 Af ter removal from the plastic bag, the sealing surface should be wiped clean with a soft, lint free cloth, using Isopropyl alchohol if necessary. Care must be taken to avoid any contamination of the pilot pneumatic feed holes on the scaling surface or the pilot vent hole in the top cover (Pc. No. 9 on DS-C-66181).

3.3 The protective plastic cap on the connector (Pc. No. 36 on DS-A-63790) should remain in place until the cable end connector is installed.

4. Removal of Existine Pilots 4.1 Bend down cl.c tabs on all pilot mounting screw locktabs (No. 14 on DS-A-63790, four (4) per pilot) until flat on the pilot bach surface.

4.2 Remove the four (4) mounting screws (Pc. No. 40 on DS-A-63790) and locktabs while holding the pilot in place.

4.3 Remove the pilot from the main valve by first sliding sideways to ensure that the two (2) "0"-Rings rer.ain in place and then pulling straight out.

4.4 Remove, clean and inspect the "O"-Rings for reuse. If the "0"-Rings are damaged or have taken a permanent set, they cust be replaced.

4.5 Inspect and wipe clean if necessary, the main valve "0"-Ring seating surfaces.

5. Installation of New Pilots l

5.1 Lightly lubricate the two (2) "0"-Rings with Parker Super-0-Lube and position l them on the mair. valve cartridge essembly. Wipe off any excess 0-Lube. i 5.2 Lightly lubricate the mounting screw threads with "Never-Seec" and place the screws and locktab washers in the four (4) mounting holes in the IMF-2 Pilot.

The locktabs should have the long tab preber.t to fit the edge of the pilot as ,

l shown on DS-A-63790.

5.3 Place the pilot onto the main valve so that the screws are aligned, and hand tighten all four (4) screws. Care must be taken to avoid excess side to side slippage or separation of the sealing surfaces to ensure the the "O"-Rings re-main properly positioned.

( 5.4 Torque the mounting screws to 120 inch-pounds using standard torquing tech-niques.

5.5 Bend up one (1) tab of each locktab washer until flush with one (1) flat on the hex head mounting screw, taking care not to damage the aluminum pilot block surface.

6. Records A record shall be made by the installing personnel of the IMF-2 serial numbers and the actuator and valve serial number upon which each pilot is installed and this record shall be included in the Field Service Report.
