PALO VERDE NUCLEAR GENERATING STATION Radiation Monitoring Unit Inoperable for Greater Than 72 Hours License No. NPF-51 Docket No. STN 50-529 Special Report No. 2-SR-87-015 This Special Report is being submitted pursuant to Technical Specification 3.3.3.1 ACTION 27 and Technical Specification 6.9.2 to report an event in which RU-148 (Containment Area Monitor Channel A) was inoperable for greater than 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />.

The 72 hour8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> limit for operability was exceeded at approximately 0045 MST on May 9, 1987. Pursuant to Technical Specification 3.3.3.1 ACTION 27 the Preplanned Alternate program to monitor the appropriate parameters was initiated.

At approximately 0045 MST on May 6, 1987, Palo Verde Unit 2 was in Mode 1 (POWER OPERATION) when RU-148 was declared inoperable in order to perform surveillance test 36ST-9SQ08 (Radiation Monitoring Calibration Test For New Scope Area Monitors). During the performance of 36ST-9SQ08 the monitor stopped operating.

Troubleshooting and repair in accordance with an approved work control document found that the communication portion of the Single Channel Area Monitor (SCAM) board was not communicating with the Remote Indicator Controller (RIC). The SCAM board was replaced and the monitor was calibrated and functionally tested in accordance with 36ST-9SQ08. A root cause analysis has been initiated to determine the cause of failure of the board.

After satisfactory completion of 36ST-9SQ08, RU-148 was declared operable at 1015 MST on May 14, 1987. The monitor was inoperable for 8 days and 9.5 hours5.787037e-5 days <br />0.00139 hours <br />8.267196e-6 weeks <br />1.9025e-6 months <br />.
