Thepurposeofthisletteris,torequestawaiverofcompliancefromTechnicalSpecification3.8.1.1(A.C.Sources-Operating)Actionb.This72-houractionwasenteredat1200hours(i.e.,noon)onAugust30,1990.Thissubmittal,evaluatesanincreaseintheactiontimefrom72hoursto15days.'tthistime,however,itisrequestedthatthewaiverincreasetheactiontimeby4days(from72hoursto7days).ThistimewillbeutilizedtoprepareanemergencyTechnicalSpecificationchangerequestthatwillrepresentPP&L'sformalrequestforthefullextensionto15days.DescritionofConditionOnAugust29,1990,duringaborescopicinspectionofEmergencyDieselGeneratorOG501D,heavyscoringof.thecylinderlinersonfiveenginecylinderswasobserved.Furtherinspectionrevealeddamagetoseveraladditionalcylinderlinersandthepresenceofaluminumoxideintheengine.Thealuminumoxidewasintroducedtotheengineasresidualmaterialfromsandblastingperformedontheengineintercoolersduringarecentmaintenanceactivity.Intakeair,passingthroughtheintercoolers,pickedupthisresidualmaterialandintroducedittotheengine.Onceintheengine,thealuminumoxidebecameanabrasiveactingbetweentheenginecylindersandcylinderliners,resultingintheheavyscoringobservedonthecylinderliners.AreviewofrecentenginelubeoilsamplestakenonOG501Drevealedthepresenceofincreasedlevelsofchrome.Thesourceandsignificanceofthisconditionwasnotimmediatelyrecognizedbutcoupledwiththeinspectionresultsnotedabove,wenowknowthesourceofthechrometobefromthescoredcylinderliners.900910034990090kPDRADOCK05000387QPDC,ok Mr.ThomasT.MartinFilesA17-2/R41-2,PLA-3438September1,1990Page2ThemaintenanceoverhaulwhichleadtotheintroductionofaluminumoxideintoOG501DwassimilartooverhaulsperformedonEmergencyDieselGeneratorsOG501A,B,andCoverthepastsixmonths.Assuch,thepotentialforasimilarconditiononthesethreeenginesexists.Theresultsofourinvestigationofthispotentialareasfollows.InvestiationResultsThesourceofthealuminumoxideinOG501Dhasbeenconfirmedtobetheintercoolersinstalledinthatengine.TheseintercoolershadpreviouslybeeninstalledinOG501A.DuringtheoverhaulevolutiontheyhadbeenremovedfromOG501A,inspectedandcleaned,(includingsandblastingthetubesideofthecoolerwithaluminumoxide)andthenreinstalledinOG501D.Precautionsagainstintroductionofaluminumoxidetotheshellsideofthecoolerwereinadequate.Thesequenceoftheengineoverhaulswas:1.OG501C2.OG501A3.OG501D4.OG501BAsimilarsequenceofintercoolerremoval,cleaningandreinstallationexistedforeachengineexceptforOG501C.Sinceitwasthefirstenginetoundergoitsoverhaul,newintercoolersdeliveredfromthevendorwerereinstalledinit.Asaresultwehavehighconfidencethatthisconditiondoesnotexist.inOG501C.TheintercoolersthatwereremovedfromOG501CwerereinstalledinOG501A.TheboroscopicinspectionthatrevealedthescoringofthecylinderlinersonOG501DhasalreadybeencompletedonOG501A.Theseinspectionsrevealednovisibledamage.Duringthisinspectionperiod,theairintakemanifoldwasremovedfromOG501Atosupportinstallationofanenginemodification.Noevidenceofaluminumoxidewaspresent.Xnaddition,tworecentlubeoilsamplestakenonOG501Awerereviewed.UnlikethesampleforOG501D,thelevelofchromepresentinthelubeoilwasnormal.Thedamagefromintroductionofaluminumoxidemanifestsitselfquickly.OG501Dhasonly20hoursofruntimesincecompletionofitsmaintenanceoverhaul.Conversely,OG501Ahas43hoursofruntimesinceitsmaintenanceoverhaulwhichwebelieveisadequatetoresultincylinderlinerscoringinthepresenceofevensmallquantitiesofaluminumoxide.ForthesereasonswehaveahighconfidencelevelthatOG501Adoesnothavethiscondition.Theintercoolersthat.wereremovedfromOG501DwerereinstalledinOG501B.Thisenginehasyettoundergoitspostoverhaulinspection.Areviewofrecentlubeoilsampleswasperformedand Mr.ThomasT.MartinFilesA17-2/R41-2,PLA-3438September1,1990Page3anincreasedlevelofchromewasobserved.Asaresult,OG501Bwasdeclaredinoperableat1200onAugust30,1990andTechSpecAction3.8.1.l.bwasenteredonbothunits.Animmediateinspectionwasperformedandevidenceofaluminumoxidewasfoundintheairintakemanifold,confirmingtheexistenceoftheconditiononOG501B.OG501E,thesparedieselgeneratoratSusquehanna,ispresentlysubstitutingforOG501D.Itwasnotpartoftherecentoverhauls.Itsintercoolershavenotbeenrecentlyremovedandsandblasted.Areviewoflubeoilsamplesonthisengineshowsnormallevelsofchrome.Basedonthisinformation,asimilarconditiondoesnotexistonOG501E.ReirementsforwhichWaiverisReestedItisrequestedthattheLCOActiontimeforTechSpecAction3.8.1.1.bonbothunitsbeextendedfrom72hoursto7daystoallowcontinuedoperationofUnit1and2.Thisextensionwillprovideuswithsufficienttimetoprepare,submit,andallowNRCreviewofanemergencyTechnicalSpecificationchangethatwilljustifytheadditionaltimenecessarytofinishperformingathoroughinspection,cleaning,andrestorationofOG501B.ComensatoActionsWehaveconfirmedthatthethreealignedDieselGeneratorsdonothavethedegradedconditionexperiencedbyOG501BandD.Assuch,theirreliabilityduringthisperiodoftimeisnotimpaired.Asnotedabove,DieselGeneratorsOG501AandChaverecentlyundergonearigorousmaintenanceoverhaulandinstallationofmodificationstoimprovetheirreliability.Thefollowingadditionalcompensatorymeasureswillalsobeperformedinsupportoftheextendedactiontime:ThespraypondbypassvalveoneachdivisionwillbemaintainedopentoassureavailabilityofDieselGeneratorCooling.2.PerTechnicalSpecification3.8.1.1,ActionB.2.,wearerequiredtoperformSurveillanceRequirement4.8.1.1.2.a.4within24hours.Itisproposedthatthissurveillancebeperformedevery72hoursthereafterforthedurationoftheextendedactiontime.Thiswillservetoprovideadditionalassuranceofthereliabilityofthethreeoperabledieselgenerators.
Mr.ThomasT.MartinFilesA17-2/R41-2,PLA-3438September1,1990Page43.Tofurtherreducetheriskofchallengestotheoffsitepowersupply,discretionarypowerchangesonbothUnits1and2willbeavoided.4~Nodiscretionarymaintenanceormodificationworkwillbeperformedonthefollowingsystems:ECCS(includingthesupportfunctionsprovidedbyESWandRHRSW),RCIC,CRDsystem,DCpowersystems,~andACPowerDistributionsystems.SafetSinificanceandConseencesof.ProosedReestTheproposedrequestisnotasignificantsafetyconcernasdescribedabove.WehavealreadyidentifiedtheuniquecauseofthisconditionandconfirmedthethreealignedDieselGeneratorsareoperable.Theadditionaltimeisneededtoassurethoroughinspection,cleaningandrestorationofOG501B.Thecompensatoryactionsoutlinedabovefurtherreducetheprobabilityofanysafetyconsequencesfromthisrequest.PP&Lhasperformedariskevaluationofa12-dayextensiontothe72-houractiontime.Thisevaluationisbaseduponthe1986IndividualPlantEvaluationforSSESthathasbeensubmittedtotheNRC.Theequationusedtoquantifytheestimatedincreaseintotalplantdamageis:Annual"'isk(1~4x10)f~+(7~0x10)kgwheref>=annualfractionwhentheA,B,CandDdieselgeneratorsareinserviceandtheEdieselgeneratorisavailabletobeputinservice(normalconfiguration),andfz=annualfractionwhenonlythreedieselgeneratorsareoperable,andoneofthethreeistheEdieselgenerator(currentconfiguration).Thevalueof1.4x10/yearisbasedontheIPEevaluationoftheaffectofopeningthespraypondbypassvalveforbothdivisions.Thevalueof7.0x10/yearisbasedontheIPEevaluationofthecurrentdieselgeneratorconfiguration.Theevaluationofthisequationforthe72-houractiontimeresultsinanannualriskfortotalplantdamageof1.45x10.For15days,theresultis1.63x10.Thisyieldsanincreaseof13%.Thisevaluationassumedthattheaforementionedcompensatoryactions1.and4.areimplemented.

AmendmentNos.51and19,datedDecember3,1985).NoSinificantHazardsConsiderations1~Thisproposaldoesnotinvolveasignificantincreaseintheprobabilityorconsequencesofanaccidentpreviouslyevaluated.ThethreeremainingDieselGeneratorswillbeoperable.ThereforeSusquehannawillbeinaconfigurationthatisboundedbypriordeterministicanalysis,assumingasinglefailurehasoccurred.Furthermore,asdescribedabove,theriskoftotalplantdamageonanannualbasishasincreased,butitisnotconsideredsignificantbecause:a.itislowerthanthetotalplantdamagefrequencyintheoriginalIPE,andb.thepercentageincreaseislowerthanwhatwasacceptedbytheNRCinapriorsafetyevaluationtoextendthesameactiontimesinordertotieinthe'E'ieselgenerator.2.3.Thisproblemdoesnotcreatethepossibilityofanewordifferentkindofaccidentfromanyaccidentpreviously'valuated.Theevolutioninvolvedisoneofrestorationtoacceptablestandards.NochangeintheoperationorfunctionoftheDieselGeneratorsisproposed.Thischangedoesnotinvolveasignificantreductioninamarginofsafety.IfanemergencyweretoariseduringtheexistingLCOortheextension,itispossibletoaddressthesafeoperationofbothunitswiththethreeremainingoperableDieselGenerators.Furthermore,compensatoryactionshavebeenproposedtoassurethereliabilityofthesedieselsandothersafetysystemswhichmitigatetheriskofaneventshouldoneoccur.Thisdoesnotrepresentasignificantreductioninamarginofsafety.

Mr.ThomasT.MartinFilesA17-2/R41-2,PLA-3438September1,1990Page3anincreasedlevelofchromewasobserved.Asaresult,OG501Bwasdeclaredinoperableat1200onAugust30,1990andTechSpecAction3.8.1.l.bwasenteredonbothunits.Animmediateinspectionwasperformedandevidenceofaluminumoxidewasfoundintheairintakemanifold,confirmingtheexistenceoftheconditiononOG501B.OG501E,thesparedieselgeneratoratSusquehanna,ispresentlysubstitutingforOG501D.Itwasnotpartoftherecentoverhauls.Itsintercoolershavenotbeenrecentlyremovedandsandblasted.Areviewoflubeoilsamplesonthisengineshowsnormallevelsofchrome.Basedonthisinformation,asimilarconditiondoesnotexistonOG501E.ReirementsforwhichWaiverisReestedItisrequestedthattheLCOActiontimeforTechSpecAction3.8.1.1.bonbothunitsbeextendedfrom72hoursto7daystoallowcontinuedoperationofUnit1and2.Thisextensionwillprovideuswithsufficienttimetoprepare,submit,andallowNRCreviewofanemergencyTechnicalSpecificationchangethatwilljustifytheadditionaltimenecessarytofinishperformingathoroughinspection,cleaning,andrestorationofOG501B.ComensatoActionsWehaveconfirmedthatthethreealignedDieselGeneratorsdonothavethedegradedconditionexperiencedbyOG501BandD.Assuch,theirreliabilityduringthisperiodoftimeisnotimpaired.Asnotedabove,DieselGeneratorsOG501AandChaverecentlyundergonearigorousmaintenanceoverhaulandinstallationofmodificationstoimprovetheirreliability.Thefollowingadditionalcompensatorymeasureswillalsobeperformedinsupportoftheextendedactiontime:ThespraypondbypassvalveoneachdivisionwillbemaintainedopentoassureavailabilityofDieselGeneratorCooling.2.PerTechnicalSpecification3.8.1.1,ActionB.2.,wearerequiredtoperformSurveillanceRequirement4.8.1.1.2.a.4within24hours.Itisproposedthatthissurveillancebeperformedevery72hoursthereafterforthedurationoftheextendedactiontime.Thiswillservetoprovideadditionalassuranceofthereliabilityofthethreeoperabledieselgenerators.

Mr.ThomasT.MartinFilesA17-2/R41-2,PLA-3438September1,1990Page43.Tofurtherreducetheriskofchallengestotheoffsitepowersupply,discretionarypowerchangesonbothUnits1and2willbeavoided.4~Nodiscretionarymaintenanceormodificationworkwillbeperformedonthefollowingsystems:ECCS(includingthesupportfunctionsprovidedbyESWandRHRSW),RCIC,CRDsystem,DCpowersystems,~andACPowerDistributionsystems.SafetSinificanceandConseencesof.ProosedReestTheproposedrequestisnotasignificantsafetyconcernasdescribedabove.WehavealreadyidentifiedtheuniquecauseofthisconditionandconfirmedthethreealignedDieselGeneratorsareoperable.Theadditionaltimeisneededtoassurethoroughinspection,cleaningandrestorationofOG501B.Thecompensatoryactionsoutlinedabovefurtherreducetheprobabilityofanysafetyconsequencesfromthisrequest.PP&Lhasperformedariskevaluationofa12-dayextensiontothe72-houractiontime.Thisevaluationisbaseduponthe1986IndividualPlantEvaluationforSSESthathasbeensubmittedtotheNRC.Theequationusedtoquantifytheestimatedincreaseintotalplantdamageis:Annual"'isk(1~4x10)f~+(7~0x10)kgwheref>=annualfractionwhentheA,B,CandDdieselgeneratorsareinserviceandtheEdieselgeneratorisavailabletobeputinservice(normalconfiguration),andfz=annualfractionwhenonlythreedieselgeneratorsareoperable,andoneofthethreeistheEdieselgenerator(currentconfiguration).Thevalueof1.4x10/yearisbasedontheIPEevaluationoftheaffectofopeningthespraypondbypassvalveforbothdivisions.Thevalueof7.0x10/yearisbasedontheIPEevaluationofthecurrentdieselgeneratorconfiguration.Theevaluationofthisequationforthe72-houractiontimeresultsinanannualriskfortotalplantdamageof1.45x10.For15days,theresultis1.63x10.Thisyieldsanincreaseof13%.Thisevaluationassumedthattheaforementionedcompensatoryactions1.and4.areimplemented.

AmendmentNos.51and19,datedDecember3,1985).NoSinificantHazardsConsiderations1~Thisproposaldoesnotinvolveasignificantincreaseintheprobabilityorconsequencesofanaccidentpreviouslyevaluated.ThethreeremainingDieselGeneratorswillbeoperable.ThereforeSusquehannawillbeinaconfigurationthatisboundedbypriordeterministicanalysis,assumingasinglefailurehasoccurred.Furthermore,asdescribedabove,theriskoftotalplantdamageonanannualbasishasincreased,butitisnotconsideredsignificantbecause:a.itislowerthanthetotalplantdamagefrequencyintheoriginalIPE,andb.thepercentageincreaseislowerthanwhatwasacceptedbytheNRCinapriorsafetyevaluationtoextendthesameactiontimesinordertotieinthe'E'ieselgenerator.2.3.Thisproblemdoesnotcreatethepossibilityofanewordifferentkindofaccidentfromanyaccidentpreviously'valuated.Theevolutioninvolvedisoneofrestorationtoacceptablestandards.NochangeintheoperationorfunctionoftheDieselGeneratorsisproposed.Thischangedoesnotinvolveasignificantreductioninamarginofsafety.IfanemergencyweretoariseduringtheexistingLCOortheextension,itispossibletoaddressthesafeoperationofbothunitswiththethreeremainingoperableDieselGenerators.Furthermore,compensatoryactionshavebeenproposedtoassurethereliabilityofthesedieselsandothersafetysystemswhichmitigatetheriskofaneventshouldoneoccur.Thisdoesnotrepresentasignificantreductioninamarginofsafety.
