This is Detroit Edison's final report on the emergency diesel generator relay failures. This problem was originally reported to Mr. P. Pelke of NRC Region III by Detroit Edison's Mr. H.A. Walker, Supervisor-Construction Quality Assurance, on February 22, 1982.
The reported failures were in the control circuitry of the division 1 emer-gency diesel generator numbers eleven and tw31ve. The ITE relays did not actually fail. However, they did not operate properly during initial start-up testing. The problem occurred when the relays were removed from a panel located in the diesel generator base, and relocated in a new panel in the second floor switchgear rom. In the process of moving the relays, the stationary contacts fell out of the relays and were improperly re-installed when they were remounted.
The electricians were trained on the proper procedure for the installation of the contacts. The relays were then reworked, inspected and retested successfully. If you have further questions, please contact Mr. G.M. Trahey, Assistant Director-Project Quality Assurance.
