-each of.two Casing Cooling Pumps was found open. These normally closed four inch valves bypass the ~ normal recirculation line. 'This event.is reportable pursuant to T.S. 6.9.1.9.b.

Probable Consequences of Occurrence The casing cooling flow available to the Outside Recirculation Spray-Pumps _would have been substantially reduced with_the recirculation bypass valves open. Since the valves were only open seven-days and the

. Unit was.in Modes 3, 4-and 5 during the period, the health and safety of the general public were not affected.

Cause of Event The valves were verified to be shut on May l'8, 1983 during the performance of two periodic tests. On May - 19, .1983 the valves were placed on their backseats by maintenance personnel to replace the packing. After maintenance the valves were _ cycled by maintenance personnel as required by the maintenance procedure. The valves were left in the open position following maintenance due to procedural inadequacy.

Immediate Corrective Action The. valves were closed immediately.

t Scheduled Corrective Action The maintenance procedure will be revised to ensure that valves are left in the correct position after maintenance.

Action Taken To Prevent Recurrence No further action is required.
