PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-V-87-32 Date: 04/23/87 This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance. The information presented is as initially received without verification or evaluation and is basically all that is known by Region V staff on this date.

SUBJECT:

FOREIGN MATERIAL FOUND IN STEAM GENERATOR At about 3:00 pm on April 22, 1987, with the unit in Mode 5 and preparing to perform selccted preventive tube plugging in the steam generators, a worker found a number of small stones in the bottom hemispherical head of the hot leg side of No. 1 Steam Generator. Scaffolding and a wooden platform had recently been erected in the hot leg side in preparation for the tube plugging activity. Similar platforms and scaffolding w:re also erected in the cold leg side, as well as on both sides of the No. 2 Steam Generator. The licensee has suspended all work activities in the steam generators until the primary side of both steam generators can be inspected. Dams were installed in the hot and cold legs close to the steam generator on April 18, prior to scaffold and platform erection, to prevent material from inadvertently entering into the reactor coolant system piping. The licensee's investigation into whether the foreign material tas unintentionally brought into the steam generator, or whether it was a deliberate act, is currently ongoing. The resident inspector is witnessing the licensee's insp;ction of the primary side of the steam generators.

Unit 3 recently completed initial fuel load and initial criticality is scheduled for July 1987.
