SUBJECT:

LOW PRESSURE SAFETY INJECTION PUMP TRIP On July 4, 1987, the low pressure safety injection (LPSI) "A" pump which was being used I for shutdown cooling, tripped from a motor overcurrent condition. The reactor was in i Mode 4 at the time. The pump had been placed inservice on July 2, 1987, replacing the LPSI "B" pump which had been placed in standby due to excessive pump seal leakage. At the time of the LPSI "A" pump trip, a smoke alarm had been received from the LPSI "A" pump room. Investigation confirmed that no fire had occurred. Following the pump trip, I the LPSI "B" pump was placed into service and the reactor placed in Mode 5. On July 5, with reactor temperature at 150 degrees F the LPSI "B" pump was shutdown andithe l containment spray (CS) "B" pump was placed in the shutdown cooling mode. '

An inspection of the LPSI "A" pump motor bearing oil reservoirs revealed the lower oil l

reservoir to be low on oil and the oil contaminated with a black foreign substance. The '

upper bearing reservoir oil level and appearance were normal. An inspection of the LPSI "B" pump lower bearing oil reservoir revealed a similar black substance in the oil, but to a lesser degree. The upper bearing oil appearance was normal. The motor and pump vendors are both onsite conducting an investigation into the LPSI "A" motor trip and the excessive "B" pump seal leakage. The licensee plans to replace the Unit 1 LPSI "A" motor with a Unit 3 CS motor which is of the same design.

Unit I has been shutdown since June 28, 1987, due to a water hammer resulting from the i closure of a discharge valve in the circulating water system. The water hammer created '

large leaks in the circulating water system line. The cause of the closure of the valve was determined to be the result of the failure of 4 bolts in the drive gear mechanism in the valve operator which allowed the valve diaphragm to rotate freely to the closed I

position. Repairs have been completed on circulating water lines. Damage to the main I

condenser internals was limited to a few leaking tubes. Steam generator ~and condenser ,

water chemistry has been upgraded through the recirculation of secondary coolant through '

the demineralization beds.  !

Unit 1 startup is contingent on the completion of repairs to the LPSI pumps.
