EMERGENCY DIESEL FIRE DURING OUTAGE (UPDATE)

The licensee has determined the cause of the 2A Emergency Diesel fire, which occurred on February 8, 1987, to be the failure of a fuel line compression fitting at the point where the fuel line was connected to the No. 5 right cylinder fuel injector. The licensee and the diesel vendor, Cooper Energy Services, have concluded that the fitting failed because it had been incorrectly assembled by a vendor employee at a vendor facili ty. The fuel line and fitting was a replacement part which had been installed -

during the current maintenance outage at Unit 2.

The licensee is performing inspections of other fuel line compression fittings to ascertain the scope of the problem. Units 1 and 2 both remain in a cold shutdown condition.

The Vendor Programs Branch has been contacted concerning the potential generic applicability of the problem. The licensee is considering submission of a 10 CFR 21 report.
