' PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE PN0-II-89-64 This preliminary notification constitutes EAPLY notice of events of POSSIBLE safety or public interest significance. The information is as initially received without verifi-cation or evaluation, and is basically all that is known by the Region II staff on this date.

SUBJECT:

UNSCHEDULED SHUTDOWN IN EXCESS OF 48 HOURS On September 7, at approximately 17:00 EDT secondary chemistry was found out of specifica-tion due to an apparent condenser tube leak. A power reduction was initiated at 1800 EDT to determine the cause of the chemistry problem.

At approximately 00:00 EDT on September 8, the licensee discovered that the MSIV "A" test circuitry was energized. Since the status of the valve was unknown, the valve was declared inoperable. Repairs to the MSIV could not be completed within the 4 hour4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> LCO, Power was reduced to less than 5 percent. Chemical analysis of the condensate system indicated a; circulating water intrusion into the condenser hotwell and the Unit was placed hot standby. Subsequent inspection of condenser and the condenser water boxes re/ealed that a 20-inch Pathway expansion bellows in the extraction steam line from the 8th stage of the I low pressure turbine 'A' had disintegrated and damaaed approximately 53 condenser tubes.

Investigation of the MSIV 'A' problem revealed that a severe water hammer occurred in the 4 inch forward feedwater flush line for steam generator A. (This system is used during startup only.) The water hammer crushed the electrical conduit containing wiring for MSIV

'A' and caused shorts which caused the test circuitry indication to illuminate. The water hammer also damaged nine supports on the forward feedwater flush line and one support for a main steam line.

The unit is currently in hot standby. Repairs are expected to take about 1 week. The licensee plans to maintain the unit in hot standby while repairs are in progress.

The resident inspector will followup.. A regional inspector will be onsite 9/12/89 to investigate the water hammer event.

Region 11 received notification of these events from the resident inspector on 9/8/89.
