This updates PNO-1-87-104 issued on November 9, 1987, which discussed Boston Edison's decision to stop work at the Pilgrim facility to allow site management the opportunity to review the causes of v m 1 problems which occurred onsite since November 7, 1987.

Among other problems wi ich principally involved the adequacy of completed maintenance work, there were four radiological matters. In one instance, on November 8, the nonnally locked high radiation barrier gate leading to the refueling floor was found with its locking mechanism taped-over. Although there existed no actual radiological .

conditions warranting the establishment of a high radiation barrier, keeping the gate locked is a current site administrative requirement. In addition, there were several radioactive sp'.11s which occurred on November 8,1987 as an apparent result of 9 incomplete maintenance or modification work activities. The contamination levels resulting from the above spills were relatively low, however, about five people received clothing and/or skin contamination and four received shoe contamination which was easily renoved onsite. Further, on November 8, 1987, operator attempts to use water to clear a clogged transfer line resulted in a spill in an unoccupied area of the radwaste facility. This spill resulted in radiation levels of 500 mR/hr on contact on the floor. Finally, the licensee identified low level contamination in a corridor adjacent to the turbine building which resulted in 12-14 people with shoe contamination.

There has been significant media interest in the events leading up to the work stoppage.
