SUBJECT:

' UNAUTHORIZED REMOVAL OF LEAD SHIELDING IN REACTOR CAVITY ACCESS AREA The licensee.was preparing to fill the fuel pool transfer canal today, when the outage manager ' assembled by a team to identify ' leaks. An auxiliary nuclear operator (ANO) was assigned to check the basement of the containment building near the reactor cavity for leaks.

'The AN0l removed lead brick shielding from the reactor cavity access in order to facilitate leak. detection. Prior to removal, the AN0's survey instrument measured dose rates of 40 millirem per hour. After removal, the dose rate instrument pegged offscale (greater

-than 5 rem'per_ hour).

The ANO re' treated and read his self-reading pocket ion chamber (PIO) which indicated a 500 millirem ' exposure. The ANO left the containment building and notified health physics.

' A health physics technician measured radiation levels at the reactor cavity access of

'50 rems per hour.

.The licensee. removed all personnel from containment until the inquiry into the event was complete and'short-term corrective action was complete.

The; worker's- TLD-was processed and indicated an exposure of 440 millirem. Workers are l allowed a quarterly exposure of at 'least 1,250 millirem without being in violation of

regulat'ory requirements.'

Region II . held a .teleconference with the licensee at 10:00 a.m., this morning and is dispatching-a radiation specialist to the site to followup on this event.

The State of Florida has been informed.

The' NRC received initial notification of this event from the resident inspector at the I

site at 8:00 a.m. (EDT) on October 9.
