NOTICE OF VIOLATION Commonwealth Edison Company Docket Nos. 50-295; 50-304 As a result of the inspection conducted on May 9 through June 29, 1989, and in accordance with 10 CFR Part 2, Appendix C - General Statement of Policy and Procedure for NRC Enforcement Actions (1987),'the following violations were identified:

1. 10 CFR 50, Appendix B, Criterion V, as implemented.bv Commonwealth Edison Company's Quclity Assurance Topical Report, CE-1 A. activities affecting quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstance.

Contrary to the above, as of March 9,1989, Zion Station did not have appropriate prescriptive instructions or procedures for so :pling of safety-related fuel oil for the emergency diesel generator storage tanks. As a result, four samples were missed during the last.two years. .

This is a Severity Level IV violation (Supplement I).

2. 10 CFR 50, Appendix B, Criterion XVI, which is implemented by the Commonwealth Edison Quality Assurance Manual, states that measures shall be established to assure that conditions adverse to quality are promptly identified, and the identification, cause, and corrective action shall be documented and reported to appropriate levels of management.

Contrary to the above:

a. Conditions adverse to quality were not promptly identified and corrected as reported in LER 304/88009 in that procedural changes to prevent a. recurrence of the reactor trip as reported in Inspection Report 295/86023(DRP); 304/86022(DRP)) have not been fully implemented. b. Conditions adverse to quality, the cause, and the corrective action taken, were not documented and reported in LER 3C4/88017 in tnat, ~ '

post-maintenance verification to prevent reversed electrical connections were not addressed. (295/89015-05(DRP);

304/89015-05(DRP))

c. Conditions adverse to c,uality, the cause, and the corrective action taken, were not documented and reported in LEP 295/89001 in that the report does not address the procedure which caused a damper to reposition nor the failure to initiate a Deviation Report. (295/89015-06(DRP); 304/89015-06(DRP))

This is a Severity Level IV violation (Supplement I).

3. Technical Specification, Administrative Controls Section 6.2.1 states that written procedures including applicable checkoff lists covering items listed below shall be prepared, implemented and maintained:

Abnormal startup, operation, and shutdown of the reactor and other systems components involving nuclear safety of the facility.

Notice of Violation 2 Zion Administrative Procedure (ZAP) 14-51-2, Rev. 4, Inspection. Test, and Operating / Tagging of Equipment, Out of Service (00S) card sheet for system 'learances are to ensure steps are performed in proper sequence.

Night or ers issued.on June 9,1989 also listed the sequence of valve manipulations for valve clearance to be performed.

Contrary to' the above, on June 9,1989, two licensed shift supervisors reviewed, and approved, an DOS card sheet with an improper sequence.

When performed, this resulted in an uncontrolled gas release.

This is a Severity Level IV violation (Supplement'I).

With respect to Item 1, the inspection showed that action had been taken to correct the identified violation and to prevent recurrence. Consequently, .

no response to Item 1 of this Notice is required, and we.have no further questions regarding this matter. With respect to the remaining items, pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for the remainin violations: (1) corrective ~ action taken and the results achieved; 2(g) corrective action to be.taken to avoid further violations; and (3) the date when full compliance will be achieved. Consideration may be given to extending your response time fcr good.cause shown.
