Commonwealth Edison Company Docket Nos. 50-295; 50-304 As a result of the inspection conducted on February 10 through March 17, 1989, and in accordance with 10 CFR Part 2, Appendix C - General Statement of Policy and Procedure for NRC Enforcement Actions (1987), the following violation was identified:
10 CFR 50, Appendix B, Criterion V, as implemented by Commonwealth Edison Quality Assurance Topical Report, CE-1-A requires that activities affecting quality be conducted using approved procedures.
Commonwealth Edison Quality Procedure 10-51 requires that in process inspections be performed at l
designated hold points according to the maintenance procedure.
Commonwealth Edison Quality Procedure 18-52, requires that QA perform surveillance of QC hold points.
Contrary to the above, on February 25, 1989, during performance of work request Z78896, QC performed an in process inspection and QA performed a surveillance of the QC hold point.
Both QC and QA signed off the hold point as acceptable without noting that weld #4 data had been recorded as weld #3.
This is a Severity Level IV violation. (supplement I) 295/89008-03(DRP)
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for this violation:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
