NOTICE OF VIOLATION Commonwealth Edison Company Docket Nos.
50-295; 50-304 i
Zion Nuclear Generating Station License Nos. DPR-39; DPR-48 During an NRC inspection conducted on November 24, 1993, through January 6, j
1994, a violation of NRC requirements was identified.
In accordance with the
" General Statement of Policy and Procedure for NRC Enforcement Actions,"
10 CFR Part 2, Appendix C, the violation is listed below:
i Title 10 of the Code of Federal Regulations (CFR), Part 50, Appendix B, l
Criterion XI, " Test Control" requires, in part, that tests be performed in accordance with test procedures which incorporate the requirements and i
acceptance limits contained in applicable design documents.
It further j
requires that test results be evaluated to ensure that test requirements are satisfied.
i Contrary to the above, as identified on November 24, 1993, the procedure chosen to monitor the degraded IC containment spray pump starting times l
(PT-6C-ST " Containment Spray C Pump System Tests and Checks") did not contain i
any requirements or acceptance criteria to evaluate the acceptability of starting time delays.
Furthermore, again as identified on November 24, 1993, the test results for the July 28, 1993, test of the IC containment spray pump, i
i were not evaluated to ensure that pump starting time requirements were met j
before declaring the pump operable.
j This is a Severity Level IV violation (Supplement I).
i Pursuant to the provisions of 10 CFR 2.201, Commonwealth Edison Company is hereby required to submit a written statement or explanation to the U.S.
Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C.
20555 with a copy to the U.S. Nuclear Regulatory Commission, Region III, 801 l
Warrenville Road, Lisle, Illinois, 60532-4351, and a copy to the NRC Resident Inspector at the Zion Nuclear Generating Station within 30 days of the date of the letter transmitting this Notice of Violation. This reply should be l
clearly marked as a " Reply to a Notice of Violation" and should include for each violation:
(1) the reason for the violation, or, if contested, the basis i
for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid f urther violations, and (4) the date when full compliance will be achieved.
If an adequate reply is not received within the time :pecified in this Notice, an order or demand for information may be issued as to why the license should 1
not be modified, suspended, or revoked, or why such other action as may be i
proper should not be taken. Where good cause is shown, consideration will be
-t given to extending the response time.
