NOTICE OF VIOLATION Illinois Power Company License No. NPF-55 As a result of the inspection conducted on January 12, 1987 through January 15, 1987, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985) the following violations were identified:

1. 10 CFR 50, Appendix E, IV. F.5 requires that weaknesses or deficiencies identified during a licensee's emergency response exercises shall be corrected.

Contrary to the above requirements, an Exercise Weakness identified in the licensee's 1985 Emergency Response Exercise, that of delaying the declaration of a General Emergency was not corrected in that a similar delay took place in the 1987 Exercise.

This is a Severity Level IV violation (Supplement VIII). =

2. 10 CFR 50, Appendix E, IV. F.5 requires that weaknesses or deficiencies identified during a licensee's emergency response exercises shall be corrected.

Contrary to the above, an Exercise Weakness identified in the licensee's 1985 Ecergency Response Exercise, that of lack of proper contamination control in the Emergency Operations Facility environmental laboratory was not corrected in that similar deficiencies were noted in tne 1987 Exercise.

This is a severity Level IV violation (Supplement VIII).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation: (1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Consideration may be given to extending your response time for good cause shown.

, Appendix EXERCISE WEAKNESSES

1. The General Emergency was not declared when required. The plant had experienced a Loss Of Coolant Accident (LOCA) with failure of the Emergency Core Cooling System (ECCS), a potential core melt situation that warranted an EAL escalation in accordance with Emergency Classification Procedure EC-02. This is a repeat of an exercise weakness identified in the previous exercise. Open Item No. 461/87003-01.
2. Contamination control practices in the E0F environmental laboratory were inadequate. The two snow samples and one air sample analyzed in the E0F environmental laboratory were essentially free of radioactive contamination, and were double-bagged to minimize external contamination potential.

However, had the samples been contaminated, laboratory and personnel contamination would have been likely for the following reasons:

a. No contamination detection instrumentation nor " swipes" were available to establish contamination status.
b. The sample bags were sealed with " duct" type tape that the technician had great difficulty removing. The violent efforts to open the bags would have lead to personnel and laboratory contamination had loose contamination been in the bags.
c. The snow samples were removed from the plastic bags by dipping containers into the bags. Dripping snow water was observed on the outside of the snow sample containers.
d. No contamination surveys appeared to be available, and no forms for survey results appeared to be available.

This is a repeat of an exercise weakness identified in the previous
