APPENDIX A j NOTICE OF VIOLATION

Boston Edison Company Docket No. 50-293 Pilgrim Nuclear Power Station License No. DPR-35 As a result of the inspection cond;cted on October 21, 1986 to November 24, 1986, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following violations were identified

A. Technical Specification 6.4, " Training", requires that fire brigade training meet or exceed the requirements of NFPA Standard No. 27-1975.

! Section 51 of NFPA Standard No. 27-1975 requires that practice drills I check the ability of members to perform the operations they are expected i to carry out with the fire equipment provided. In addition, Section 52 j of NFPA Standard No. 27-1975 states that during drills, equipment should i be operated whenever possible.

. Contrary to the above, on November 4, 1986, a practice drill did not '

adequately check the ability of fire brigade members to perform the operations they are expected to carry out with fire equipment provided.

Specifically, no fire hoses or extinguishers were broken out and the brigade was not required to demonstrate other fire fighting duties, e.g.,

contacting the control room, summoning assistance to help. evaluate the extent of the fire, or simulating the containment of fire generated
radioactive contamination.

This is a Severity Level IV Violation (Supplement I).

I B. Pilgrim Nuclear Power Station Technical Specification 6.8.A states that  !

exceed the requirements of section 5.1 of ANSI N18.7-1972. Section 5.1 1 of ANSI N18.7-1972, Administrative Controls for Nuclear Power Plants, 1

requires that modifications that may affect functioning of safety-related 1 systems or components be performed in a manner to assure quality. ANSI l N18.7 further requires that major modifications be performed in accordance with applicable design requirements, specifications and inspection j requirements.

Contrary to the above as of November 21, 1986 safety related modifications were not performed in a manner to assure quality and were not performed in accordance with applicable design requirements. In addition, post work 1

inspections which were conducted were ineffective. Specifically, during implementation of Plant Design Change 83-51:

j (1) Ten electrical tie-ins to safety-related equipment were not properly implemented in that leads were landed on incorrect points on

Appendix A 2 (2) Procedurally-required post work inspections did not identify the improper tie-ins.

As a result, the final configuration was not in accordance with approved design drawings and the prescribed post work testing was found to be invalid.

This is a Severity Level IV Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Boston Edison Company is hereby required to submit to this office within thirty days from the receipt of the letter which transmitted this Notice, a written statement or explanation in reply to Violation B, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time. No response to Violation A is required at this time, provided that the corrective actions in section 3 of the attached inspection report are completed by the stated dates. If different corrective actions or completion dates are anticipated, you should inform this office within thirty days of receipt of the letter that transmitted this Notice.
