APPENDIX A NOTICE OF VIOLATION' Boston. Edison Company. Docket No. 50-293 Pilgrim Nuclear Power Station License No. DPR-35 Plymouth, Massachusetts AIs.a result of the inspection conducted at Pilgrim Nuclear Power Station from ~

May 25 through July 10, 1989 and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following violations were identified:

A. Technical Specification 3.2.B " Core and Containment Cooling' Systems -

Initiation and Control," states that the limiting conditions for operation

~ for the instrumentation that initiates or controls the. core.and contain--

ment cooling systems are given -in Table 3.2.B. This instrumentation must-be-operable.when the system (s) it initiates or controls are required to be operable.

Technical Specification Table 3.2.B, " Instrumentation -that Initiates or Controls the Core and Containment Cooling Systems," states that the mini-mum number of operable instrument channels per trip system for the RCIC turbine compartment wall temperature detection function is two.

Contrary to the above, from May 25, 1989 to June 9, 1989, one trip system had only one instrument channel operable for the RCIC turbine compartment wall temperature detection function. This instrumentation controls the Primary Containment /RCIC system turbine steam supply isolation valves, which were required to be operable during that time.

This is a Severity Level IV Violation (Supplement I).

B. Technical Specification 6.8.A requires that written procedures and admini-strative policies be established and implemented that meet or exceed the requirements of Section 5.1 of ANSI N18.7-1972.

ANSI N18.7-1972, Section 5.1, " Rules of Practice," states that policies shall be provided to control the issuance of documents that prescribe ac-tivities affecting safety-related systems or components. These policies shall assure that documents are reviewed for adequacy.

Contrary to the above, on June 22, 1989, procedure TP 88-78, " General Electric CR 2820 Time Delay Relays for ADS, CS, RHR and RPS Systems," was ,

approved without proper review for adequacy, in that, as found during per- ,

formance of the test on June 24, 1989, the approved procedure had incor-rect relay numbers in steps 3.7.A and 4.8.A of Attachment 3.

This is a Severity Level IV Violation (Supplement I).

Appendix A 2 C. 10 CFR 50 Appendix B, Criterion V, Instructions, Procedures and Drawings, and Boston Edison Company Quality Assurance Manual (BEQAM) Volume II Sec-tion 5, Instructions, Procedures and Drawings, require that quality-related activities shall be governed by appropriate instructions, proce-dures and drawings. BEQAM Section 5.2.7 states that all maintenance and modification activities affecting station structures, systems and com-ponents are to be controlled by the Maintenance Request System. Further, BEQAM Section 14, Inspection, Test and Operating Status, Step 14.2.3 re-quires that controls be established for the application and removal of status indicators such as tags, markings, labels and stamps to assure that the inspection, test and operating status of structures, systems and com-ponents is clearly indicated.

Contrary to the above, on May 30, 1989 the licensee replaced equipment and control identification name plates on control room front panels without a maintenance request or procedural controls which provided for authoriza-tion, review or verification of the work activity.

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Boston Edison Company is hereby required to submit to'this office within thirty days of the date of receipt of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
