.O APPENDIX A NOTICE OF VIOLATION

-Boston Edison Company Docket No. 50-293 Pilgrim Nuclear Power Station License No. DPR-35 As a result of the inspection conducted on February 21, 1987 to April 6, 1987, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following violation was identified:

Technical Specification 6.8.0 states that written procedures to implement the Fire Protec .fon Program shall be established, implemented and main-tained. Nuclear < eganization Procedure N0P 83FPI, PNPS Fire Protection Program, states tt.at all fire protection systems and equipment are tested, inspected, surveyed and maintained to ensure that they can perform their intended functions. In addition:

Procedure 2.1.12, Revision 13, Daily Diesel Generator Surveillance, and monthly surveillance procedure 8.A.6, Revision 14, Fire Exting-uisher. Quick Checks and Maintenance Inspection, verify that the diesel generator room dry chemical system pressures are acceptable.

Procedure 2.1.12 also verifies that the B diesel generator room sprinkler supply system heat tracing temperature is acceptable.

Procedure 8.8.22, Revision 5, Halon 1301 System-Cable Spreading Room, Attachment A, checks as-found halon storage cylinder pressures and temperatures, calculates required full charge pressure values, and assesses the acceptability of cylinder pressure.

Contrary to the above, between January,1985 and February, 1987, estab-lished fire protection equipment test, inspection and surveillance pro-cedures were not properly implemented. Specifically:

Dry chemical extinguishing system cylinder pressures were incorrectly assessed as acceptable during the daily and monthly performances of surveillance procedures 2.1.12 and 8. A.6 between December 23, 1986 and February 19, 1987. In fact one cylinder had been below the min-imum pressure limit since December 23, 1986. Improper performance of these surveillances resulted in the failure to recognize that the system was inoperable for eight weeks.

Appendix A 2 The B diesel generCor room sprinkler supply system heat tracing temperature was incorr?ctly assessed as acceptable during performance of surveillance procedure 2.1.12 on numerous occasions between January, 1985 and February,1987. In fact, the heat trace tempera-ture could not be checked because the temperature indicator had been missing since January 14, 1985. Improper performance of this sur-veillance resulted in failure to adequately verify heat tracing operability for over two years.

Cable spreading room halon system cylinder full charge pressure values were not properly calculated during performance of surveil-lance procedure 8.B.22, Attachment A, on January 29, 1987. As a result, cylinder pressures were not adequately assessed.

This is a Severity Level IV Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Boston Edison Company is hereby required to submit to this office within thirty days from the receipt of the letter which transmitted this Notice, a written statement or explanatf or. in reply, including: (1) the corrective steps which have been taken and the re-suits achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
