APPENDIX A NOTICE OF VIOLATION Boston Edison Company M/C Nuclear Docket No. 50-293 Boston, Massachusetts 02199 License No. DPR-35 As a result.of the inspection conducted on January 5-9, 1987, and in accord-ance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR 2, Appendix C, (Enforcement Policy 1986), the following viola-tion was identified:

1. 10 CFR Appendix B Criteria XVI requires that measures be established so that conditions adverse to quality such as failures are corrected. In 1984 the Post Accident Sampling System (PASS) piping had leaked due to cracking caused by too high a heat tracing temperature and internal halogen contamination. Procedure 2.2.133 requires a heat tracing tempe- I rature of 250*F i 20*F for the PASS piping.

Contrary to the above, on January 7, 1987 the temperature measuring instrumentation for the PASS piping heat tracing indicated a temperature of 364 *F.

This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, Boston Edison Company M/C Nuclear is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
