SUBJECT:

EA 96-127: St. Lucie PIG Inoperability The following is my list of ' barriers" which failed in this event, transcribed from my enforement panel notes for purposes of legibility (in other words, I  !

The HP technician failed to have the procedure in hand as required Te HP technician failed to properly reposition the valve The operators failed to report the out-of-spec surveillance data The SR0s didn't perform an adequate review of the surveillance data The procedure for hahdling out-of-spec surveillance data was ,

inadeuqate, in that it specified use of hard copy rather than use of hand-help computers The flow indicator alarm appeared to have the wron~g setpoint i As a result of these failures:

TS Violation for mode change while LC0 applied Following mode change, failed to comply with action statement (See ,

LER)

Inadequacies in licensee corrective action:

Did not counsel the operators, only the HP technician Did not review logs for other examples of data outside of acceptance criteria but not identified and addressed  ;

Did not address potential generic issue of other possible failure to declare equipment inoperable during tests such as this (this one was identified by residents and documents in IR and EAW)

Points which give this event extra regulatory significance:

Repeat of other violations (escalated issues?), which would be possible grounds for escalating this issue The general ongoing theme of procedural adherence problems The fact that several different crews did the same thing.
