I'll be on leave next week. That coincides with the SSFI bagma.n trip and Fred Wright's inspection. If this is a problem, let me know.

I'll be on leave for a couple of weeks at the Christmas holiday.

Joel will be rotating to NRR in January for two months. We need to find someone to act for him. Steve Sandin (AEOD), who has spent a lot of time here and who's a very good

' inspector, has indicated that he may be able to pick up a month of it, but we may need you to talk to his boss.

. Crystal River - Failure to perform an adequate 10 CFR 50.59 evaluation 6n diesel generator loading following a modification to remove the automatic start of the turbine i

driven auxiliary feedwater pump.

Crystal River - EFW susceptibility to single failure: during LOOP a failure of one fuse could cause an EFW flow control valve to fail open allowing one EFW pump to go.to

runout, Since both EFW pumps draw suction through the same pipe, the resulting flowrate would cause cavitation and failure of both EFW pumps.

c)

St Luc'ie/ Turkey Point - A vulnerability was identified in the safeguards procesC for FPL where the access authorization was not immediately removed for individuals whose

-employment was terminated. One individual after employment termination reentered the

St Lucie - The EP inspection identified potential multiple violations. The violations were in the areas of' timely activation of response facilities. training'of respons'e personnel and corrective actions for 1995 Hurricane Erin critique items.

. PLANT ISSUES / EVENTS:

- a).

.St Lucie - Both units,are operating at 100%.

b)

Turkey Point - Both units are operating at 100%.

Unit 4 had a ~ feed water heater. relief lift prematurely due' to a :know design' flaw.

The spring is susceptible to hydrogen embrittlement.. The relief valve was like kind replaced with the unit at 77% power.

This problem does not effect Unit 3.. Unit-4 will be taken off line in about a-week to replace the springs on all 8 of the effected relief valves.

Unit 3 and ~4 received the license amendment allowing thermal uprate of approximately 5% power.

Unit 3 is performing all the equipment calibrations to support the upr. ate with plans to next week actually raise power to the new 2300 Megawatts thermal.

Unit L3 and 4 reported a procedural error which 'would have allowed a potential uncoiltrollable radiological rel. ease to the environment.(see the attached 50 72).

c)

Crystal River - The unit remains shutdown pending a license amendment to resolve several US0s on EDG loading and Emergency Feedwater (EFW) susceptibility to single failure.

FPC plans to submit a remain shutdown for another two months or more to resolve the design issues and submit the necessary license amendments (see attached PN).

An " unusual event" was declared on October 7. 1996, at 8:49 a.m. because

'of a hurricane warning. issued by the State of Florida. At approximately 11:00 p.m. on October 7.1996. the National Hurricane Center lifted the hurricane warning for the Crystal River area. At that time the licensee exited the Unusual Event (see the attached PNs).

The investigation by the' licensee into the apparent tampering event -

associated with the "A"

EDG was completed on 10/03/96.

The licensee

. concluded that poor work practices caused-strairier blockage and has secured from the security alert.

Billy Crowley revie.ved the report on 10/7/96 and exited with the--licensee on 10/9/96.
