FINAL REPORT - REVISION 1 - DER 80-23 DEFICIENCY EVALUATION 50.55(e) r ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNIT 1, 2 & 3 I. DESCRIPTION OF DEFICIENCY d After receipt of the Callaway/ Wolf Creek 50.55(e) report concerning Corner & Lada (C&L) sway struts, dated July 24, 1980, an inspection was initiated to review the size 1, 2, and 3 sway struts delivered to the PVNGS jobsite. The identified condition is loose bushings in the sway struts at the pipe clamp end which can result in the t paddle becoming completely disengaged from the bushing, causing a 3/8 inch gap between the paddle and the pin.

Upon receipt of Bechtel Quality Assurance Bulletin 80-30 and Bechtel Problem Alert TPM-P-80-6, October 13, 1980, which references a 10CFR Part 21 Report filed by Bechtel's Ann Arbor

, Office concerning a generic industry-wide concern for these products and installations, this project expanded the scope of this problem to include the installation of all C&L sway struts.

However, no deficiency has bee.- observed or reported by other sources concerning'the t es 4 t. cough 9 sway struts. For sizes 4 through 9 sway struts, CdL uses a different type of operation for installing the bushings than for sizes 1, 2 and 3 .

Subnequently, Corner & Lada and Bechtel's Gaithersburg office performed a complete review of the size 1 sway struts and concluded that it i dimensionally impossible for the bushings in size 1 to become completely dislodged.

g II. ANALYSIS OF SAFETY IMPLICATIONS This condition is considered to be Reportable concerning C&L size 2 L

and 3 sway struts, since if left uncorrected, the existence of the

- _l8 inch gap between the paddle and pin duriub a seismic event may result in piping and/or piping welds being overstressed due to a

movement of the piping system.

III. CORRECTIVE ACTION

1. Bechtel has verified that C&L instituted a shop procedure which requires staking of all size 1, 2 and 3 rigid sway strut rod paddle ends. This operation eliminates any

possiblility of disengagement of the spherical bushing from end assembly. This ,rocedure began on November 19, 1979.
