Interim Report of 10CFR50.55(e) Item on Failure Main Steam Isolation Valves (#50)

On February 17, 1982, Detroit Edison's Mr. H. A. Walker, Supervisor-Construction Quality Assurance, telephoned Mr. P. Byron of NRC Region III to report on a problem with failure of Main Steam Isolation V.-Lves at the Fermi 2 site.

Four motor operated Main Steam Isolation Valves located in the Turbine Building had failed to operate due to improper assembly. This improper on-site ascembly of the Belleville washers on the packing retainer caused the valve stem to bind and did not allow the valve to open or close. These isolat 'on valves are the third valves in the series away from the iteactor which were added per NRC requirements.

Detroit Edison is investigating the cause of the improper assembly and wi.11 take the necessary corrective action of training of personnel and/or changing of procedures covering the valve assembly to prevent recurrence of the problem.

Another report on this item, either interim or final, is scheduled to be sent to you on or before July 16, 1982.
