This action violated Technical Specification requirement 6.2.2.b, which requires at least one Senior Operator present in the control room while the unit is in Mode 1, 2, 3, or 4. The cause of this event was inappropriate action by the Unit Supervisor. Corrective actions for this event include a Human Performance Review, verbal discussions on the command and control function requirements by Management, and procedures will be revised to i add a required discussion concerning command and control function for l License Activation Certification. This event will also be included in j Licensed Operator Requalification Training.

On February 14, 1993, at 0433 hours0.00501 days <br />0.12 hours <br />7.159392e-4 weeks <br />1.647565e-4 months <br />, Unit 2 was in Mode 4 at 0% t power. The on-duty Shift Supervisor designated the Unit Supervisor as the individual responsible for the control room command function and exited the control room. The Unit Supervisor, the only Senior Operator in the control room, exited the control room to observe a portion of a surveillance in progress in the relay racks adjacent to the control room. Upon realizing his error, the Unit Supervisor immediately returned to the control room. Security records were reviewed and it was determined that the control room had been unmanned by a Senior Operator for a period of 41 seconds. During this period of time, two reactor operators remained in the control room. The NRC was notified of this event on February 14, 1993, at 1108 hours0.0128 days <br />0.308 hours <br />0.00183 weeks <br />4.21594e-4 months <br />.

I The Technical Specifications, section 6.2.2.b, requires that a '

Senior Operator shall be in the control room with the Unit in Mode 1, 2, 3, or 4. Technical Specification Table 6.2-1, specifies that during the ab'ence s of the Shift Supervisor from the control room while the unit is in Mode 1, 2, 3, or 4, an individual (other than the Shift Technical Advisor) with a valid Senior Operator license shall be designated to assume the control room command function.

CAUSE OF EVENT:

The cause of this event was inappropriate actions by the Unit Supervisor. The Unit Supervisor was cognizant of his responsibility to maintain control room command function in the absence of the Shift Supervisor. However, his preoccupation with the surveillance in progress caused his loss of focus on his primary responsibility, resulting in a violation of Technical Specification.

This event is reportable pursuant to 10CFR50.~73 (a) (2) (1) (B) . '

There were no adverse radiological or safety consequences as a result of this event.

CORRECT 1VE ACTIONS:

1. A Human Performance Review Board was conducted with the Unit Supervisor and representatives from Operations, Training, Corrective Action Group, and Management. As a result, the '

Plant Manager will discuss the command and control function requirements with Licensed Senior Operators filling the i positions of Shift Supervisor and Unit Supervisor. These discussions will be completed by June 11, 1993.

2. The Licensed Operator License Maintenance procedure will be .

revised to add a required discussion between Licensed Operators and Plant Operations Manager concerning command and  ;

control functions. These discussions will be held prior to License Activation Certification. The procedure revision will be completed by August 9, 1993.

3. This event will be included in the lessons learned portion of Licensed Operator Requalification Training Program by July 16, 1993.

f ADDITIONAL INFORMATION:

There have been no similar events reported to the NRC involving a violation of Technical Specification 6.2.2.b.
